体外受精-胚胎移植术后多胎妊娠的预防及早期护理

The prevention and early nursing of multiple pregnancies treated with in vitro fertilization and embryo transfer

目的：探讨体外受精－胚胎移植术（IVF－ET ）后多胎妊娠的预防和早期护理措施。方法选择我科2014年1月～12月行IVF－ET术后多胎妊娠的孕妇230例为研究对象，采用减少胚胎移植数预防多胎妊娠，在多胎妊娠孕早期予以减胎措施。通过予以适宜的预防和孕早期保胎护理干预，同时注重双胎继续妊娠的护理，以期降低母婴并发症及围产儿死亡率，提高新生儿足月分娩率。结果经过减少胚胎移植数及孕早期的护理，发生早中期流产、引产12例，占5．22％；早产110例，占47．82％；足月分娩64例，占27．82％；多胎手术及自然减胎44例，占19．13％；新生儿存活385个。结论多胎妊娠孕妇通过有效的预防、控制和护理，提高了保胎率、足月分娩率及新生儿存活率。

Objective To investigate the appropriate prevention and early nursing of multiple pregnancies treated with in vitro fertilization and embryo transfer （ivf -et） .Methods A total of 230 pregnant women diagnosed multiple pregnancies treated with ivf -et were collected to research subjects from Jan .to Dec .2014 .The decrease of embryo transfer number was applied to prevent the multiple pregnancies .And the fetal reduction was used to multiple pregnancies during early pregnan‐cy .The appropriate methods of prevention and early nursing were applied and the twin pregnancy was also focused on nurs‐ing methods ,in order to reduce the maternal complications and reduce perinatal mortality ,the rate of childbirth full-term newborn .Results After nursing ,reduce the number of embryo transfer and the early stages of pregnancy occurs early met‐aphase abortion ,induced labor in 12 cases ,accounting for 5 .22% .Preterm birth in 110 cases ,accounting for 47 .82% . Term 64 cases ,accounting for 27 .82% .Fetal surgery and more natural reduction in 44 cases ,accounting for 19 .13% ;385 newborn survival .Conclusion the patients with multiple pregnancies through effective prevention ,control and care ,child‐birth spuc rate ,term and neonatal survival rate .
