“一居委一医生一护士”在社区老年慢性病护理新模式的应用研究

Application of “a Wei a doctor a nurse” on a New Model of Chronic Care in the elderly of Community

目的探讨"一居委一医生一护士"在社区老年慢性病护理新模式的应用效果。方法收集我社区老年慢性病患者200例,按是否愿意接受"一居委一医生一护士"护理新模式干预分为对照组和实验组,各100例。对照组给予常规社区护理。实验组采用"一居委一医生一护士"护理新模式。比较两组生活质量及平均血糖、血压的差异。结果两组干预后人际关系、环境支持、生理状况、心理状态评分比较有显著性差异(P<0.01);两组干预后平均血糖、血压值比较有显著性差异(P<0.01)。结论 "一居委一医生一护士"在社区老年慢性病护理新模式的应用效果显著,具有社区护理的借鉴性。

Objective To explore＆quot;a Wei a doctor a nurse＆quot;application results on a new model of community care of the Elderly .Method 200 cases of my community of senile chronic diseases patients according to whether they are willing to ac‐cept ＆quot;a Wei a doctor a nurse＆quot;intervention model were divided into two groups .Patients of the control group were treated with routine community care ,the experimental group were implemented ＆quot;a Wei a doctor a nurse＆quot;new model of care .Com‐pared the ,quality of life and mean blood pressure ,blood sugar between the two groups .Result The differences of patients relationships ,environmental support ,the presence of post-intervention between physiology and mental state in two groups were statistically significant （P〈 0 .01） .After the intervention ,the average blood glucose ,mean blood pressure in two groups were significant difference . It has statistically significant （P〈 0 .01 ） . Conclusion ＆quot;A Wei a doctor a nurse＆quot; in the application of the new model of the effect of aged care chronic disease significantly ,with community care Reference .
