基于信息、管理、关系框架的延续护理在维持性血液透析患者中的应用研究

Application of Extended Care on the Maintance Hemodialysis Patients Based on Information,Management and Relations

目的：观察基于信息、管理、关系框架的延续护理在维持性血液透析患者中的应用效果。方法选择2014年1月～2015年3月在我科行首次血液透析的122例维持性血液透析患者作为研究对象，随机分为对照组和试验组，各61例。对照组接受常规延续护理。试验组接受基于信息、管理、关系框架的延续护理。并对两组干预后的各项观察指标进行比较分析。结果试验组干预后自护能力评分和生活质量评分均显著高于对照组，并发症发生率显著低于对照组（P＜0．05）。结论对维持性血液透析患者实施基于信息、管理、关系框架的延续护理，有利于提高其自护能力，降低并发症发生率，改善患者生活质量。

Objective Observation extended care based on information ,management and relations in the framework of the application results in maintenance hemodialysis patients in .Methods Select 122 cases of hemodialysis patients on main‐tenance hemodialysis who were hemodialyzed from January 2015 to March for the first time in our department as subjects . The patients were randomly divided into a control group and the test group .61 patients in the control group received conven‐tional extended care ,while in experimental group received based on extended care information management ,relations frame‐work .Then compare the observed indicators for two maintenance hemodialysis patients after the intervention .Results Self-care ability score and quality of life scores in the experimental group after the intervention of hemodialysis patients were significantly higher than the test group ,while the incidence of complications was significantly lower （P〈0 .05） .Conclusion On maintenance hemodialysis for patients with extended care based on information ,management ,relationship framework will help to improve their self -care capacity ,reduce the likelihood of complications and improve quality of life .
