心脏收缩调节器的围手术期程控二例:避免与心脏其他植入器械的相互干扰

摘要 病例1为心脏再同步化治疗复律除颤器(CRT-D)术后无反应患者,植入心脏收缩调节器(CCM)后,器械间出现干扰,CRT-D程控发现心房远场感知了CCM信号,从而启动自动模式转换,导致双室起搏比例下降。将心室后心房空白期由100ms延长为200ms后,CCM信号不再被心房感知,CRT-D和CCM均工作正常,未再发生相互干扰。病例2为埋藏式心脏转复除颤器(ICD)术后患者,CCM植入术中调试时发现器械间出现干扰,ICD程控发现CCM序列延迟为100ms时,腔内心电图与原窦性心律模板匹配度为0。将CCM序列延迟调整为40 ms,腔内心电图与原窦性心律模板匹配度为100%,此时ICD和CCM均工作正常,未发现相互干扰。
