慢性肝炎患者肝功能、胃泌素和胃动素表达与胃黏膜病变的相关性研究

The correlation of liver function,gastrin and motilin's expression with gastric mucosal lesions in patients with chronic hepatitis

[目的]探讨慢性肝炎患者肝功能、胃泌素和胃动素表达在胃黏膜损伤中的临床意义。[方法]收集2013年5月~2015年7月我院消化内科诊治的慢性肝炎患者126例。电子胃镜检查胃黏膜损伤,ELISA法检测ALT和AST肝功能指标,放射免疫法检测胃泌素、胃动素表达。[结果]轻度、中度和重度慢性肝炎患者胃黏膜损伤的胃镜检出率分别为94.7%（36/38）、96.2%（50/52）和97.2%（35/36）,各组之间差异无统计学意义（P〉0.05）。但胃黏膜损伤程度表明,轻度、中度和重度慢性肝炎患者之间胃黏膜损伤程度差异有统计学意义,胃黏膜损伤程度与临床分级呈显著正相关（r=0.416,P〈0.05）。不同程度胃黏膜损伤患者之间ALT和AST肝功能指标以及胃泌素和胃动素表达差异有统计学意义（P〈0.05）。其中重度患者高于中度患者（P〈0.05）,中度患者又高于轻度患者（P〈0.05）。[结论]慢性肝炎患者的肝功能损伤、胃泌素和胃动素高表达参与了胃黏膜病变的形成。

[Objective]To explore the clinical significance of liver function,gastrin and motiin expression in gastric mucosal lesions in patients with chronic hepatitis.[Methods]One hundred and twenty-six patients with chronic hepatitis were enrolled from May 2013 to July 2015 in this study.Gastric mucosa lesion was detected by electronic gastroscopy.The expression of ALT and AST was detected by ELISA analysis.The gastrin and motilin expression was detected by radioimmunity analysis.[Results]The rate of gastric mucosal lesions in patients with mild,moderate and severe chronic hepatitis was 94.7%（36/38）,96.2%（50/52）and 97.2%（35/36）respectively.There was no significant difference between the groups（P〈0.05）.But the degree of gastric mucosal lesion showed significant difference between mild,moderate and severe chronic hepatitis patients.And the degree of gastric mucosal lesion was significantly positive correlated with the clinical chronic hepatitis classification（P〈0.05,r=0.416）.ALT,AST,gastrin and motilin showed significant difference among different degree of gastric mucosal lesion（P〈0.05）.The indexes were the highest in the severe patients,then the moderate patients,the lowest in the mild patients（P〈0.05）.[Conclusion]The liver function damage and high expression of gastrin and motilin in patients with chronic hepatitis were involved in the formation of gastric mucosal lesions.
