三维适行放射治疗食管癌疗效相关性分析

Related factors of three-dimensional conformal radiotherapy for esophageal cancer

[目的]观察中晚期食管癌患者三维适行放射治疗（3DCRT）疗效并对相关因素进行分析。[方法]收集2010年1月~2011年12月我院130例食管癌患者临床资料,采用3DCRT技术,常规剂量分割,全组处方剂量为56~68Gy/6~7周,中位剂量64Gy。观察近期疗效、急性毒副反应及1、2、3年生存情况,并对相关因素进行分析。[结果]3DCRT食管癌的近期有效率为94.6%;其1、2、3年肿瘤局控率分别为79.2%、67.7%、53.1%;1、2、3年生存率分别为73.8%、54.6%、32.3%;仅9.2%患者发生3级以上放疗反应。[结论]3DCRT是一种高精度治疗技术,明显提高中晚期食道癌局部控制率和近期疗效,优于常规放射治疗,且不良反应发生率低。

[Objective]To observe the clinical efficacy and analyze the related factors of three dimensional conformal radiotherapy（3DCRT）for middle-advanced stage esophageal cancer.[Methods]From January2010 to December 2011,130 cases of esophageal cancer patients were treated with 3DCRT,conventional dose fraction,the prescribed full dose was 56 to 68Gy/6~7weeks,median dose was 64 Gy.The toxicity,short-term effect,and the survival rate of 1,2 and 3-year were analyzed.[Results]The short-term effective rate of the 130 cases received 3DCRT is was 94.6%.The local control rates of 1,2and 3-year were 79.2%,67.7%,53.1%respectively.The survival rates of 1,2and 3-year were 73.8%,54.6%,32.3% respectively.The occurrence rate of radiotherapy reaction above the level of 3 was 9.2%.[Conclusion]3DCRT was a high-precision technology which could significantly improve the short term efficacy and local control rate of the patient with middle-advanced esophageal stage esophageal cancer,showing better efficacy than the conventional radiation therapy with lower occurrence rate of adverse reactions.
