血清CA199检测在肝硬化中的临床价值

Clinical value of serum CA199 detection in patients with hepatocirrhosis

[目的]探讨血清CA199的检测在肝硬化中的临床价值。[方法]检测72例对照组与1210例肝硬化患者的血清CA199，并分析血清CA199水平变化与肝硬化患者的预后、肝功能损害程度以及肝性脑病等的关系。[结果]肝硬化组血清CA199水平[41．8（1．6～1998．O）U／m1]显著高于对照组C8．5（O．8～37．O）U／m1]，z＝10．89，P〈0．01。肝硬化患者血清CA199水平死亡者[90．0（2．7～1983．0）U／roll显著高于存活者[40．7（1．6～1998．0）u／m1]，z＝5．93，P〈O．01；肝功能B级者E44．5（1．7～1998．0）U／m1]显著高于肝功能A级者E30．1（1．6～942．3）U／m1]，Y。=38．06，P〈O．01，肝功能C级者E80．3（2．7～1998．0）U／m1]显著高于肝功能A级者E30．1（1．6～942．3）U／ml，x2=143．00，P〈0．013、B级者[-44．5（1．7～1998．O）U／m1，x2＝46．45，P〈O．01]；并发肝性脑病者[-70．8（1．8～1983．O）U／m1]显著高于未并发肝性脑病者[39．6（1．6～1998．0）U／m13，z＝5．35，P〈0．01。[结论]肝硬化患者血清CA199水平明显升高，并与肝硬化患者的预后、肝功能损害程度及肝性脑病等因素有密切关系。

[Objective]To study the changes of serum CA199 and its clinical value in patients with hepa- tocirrhosis, f-Methods]The serum CA199 in 72 cases of normal controls and 1210 cases with hepatocirrhosis were detected. The relationship between serum CA199 and the factors of prognosis, degree of hypohepatia, hepatic encephalopathyin hepatocirrhosis patients were analyzed and compared with the normal control [Results]The serum CA199 in hepatocirrhosis group（41. 811.6-1998.0]U/ml）was significantly higher than that in control group（8.5[-0.8-37.0]U/ml） （Z= 10.89,P〉0.01）. The serum CA199 in patients with death（90.0[2.7-1983.0]U/ml）,the degree of liver function Child-pugh B（44. 511.7-1998.0]U/ ml） ,the degree of liver function Child-pugh C（80.3[-2.7-1998.0]U/ml）,complicate hepatic encephalopa- thy（70. 811.8-1983.0]U/ml）were significantly higher than the survival patients（40.7 [1.6-1998.0]U/ ml） （Z=5.93 ,P〈0.01）, the degree of liver function Child-pugh A（30.1[-1. 6- 942.3]U/ml） （Z2 = 38. 06, P〈0.01 ）, the degree of liver function Child-pugh A（30.1 [ 1.6 - 942.3 ] U/ml） （Zz = 143.00, P〈0.01 ） and B（44.5[-1.7-1998.0]U/ml）（ 2=46.45,P〈0.01）,not complicate hepatic encephalopathy（39. 611. 6- 1998.0]U/ml）（Z=5.35, P〈0.01）, respectively. [Conclusion]The serum CA199 was obviously increased in hepatocirrhosis,and it is closely correlated to the factors of prognosis, degree of hypohepatia, whether complicated hepatic encephalopathy,etc.
