Ho/Nd:YAG双波长激光术治疗高龄高危前列腺增生的疗效观察

Observation on curative effects of Ho/Nd: YAG double wave-length laser surgery in the treatment of senior and high risk benign prostatic hyperplasia

目的探讨Ho/Nd：YAG双波长激光术治疗高龄高危前列腺增生（BPH）的疗效及安全性。方法应用Ho/Nd：YAG双波长激光手术治疗高龄高危BPH患者43例,记录患者术前、术中及手术后随访资料,分析评价其疗效及安全性。结果 43例手术均获成功。手术时间30~110（63.37±22.70）min,术中出血量10~100（45.79±20.51）ml,术后留置导尿管时间23~48（25.84±4.82）h,持续膀胱冲洗时间10~48（23.28±6.73）h,术后住院时间1~2（1.19±0.39）d,术后3~6个月随访国际前列腺症状评分（IPSS）、生活质量指数评分（QOL）、最大尿流率（Qmax）及残余尿量（PVR）较术前明显改善（P〈0.05）。术后并发症发生率低。结论 Ho/Nd：YAG双波长激光术治疗高龄高危BPH具有微出血、留置导尿管、膀胱冲洗及住院时间短等优势,是可选择的安全有效的治疗方法之一,值得临床进一步推广。

Objective To explore the curative effects and safety of Ho/Nd： YAG double wave-length laser surgery in treatment of senior and high risk benign prostate hyperplasia. Methods Ho/Nd： YAG double wave-length laser surgery was used to treat 43 cases of senior and high risk benign prostatic hyperplasia. Preoperative, intraoperative and postoperative follow-up information of these patients were recorded to assess the curative effects and safety of the method. Results All of the 43 cases of surgeries were successful. The operation time was 30~110（63.37 ± 22.70）min, intraoperative blood loss was 10~100（45.79 ± 20.51） ml, postoperative catheterization time was 23~48（25.84 ±4.82） h, postoperative bladder irrigation time was 10~48（23.28 ± 6.73） h and postoperative hospitalization stay was1 ~2（1.19 ± 0.39） d. Significant improvements were achieved in IPSS, QOL, Qmaxand PVR in postoperative 3 ~6months compared with preoperative values（P〈0.05）. Postoperative complication rate was low. Conclusions Ho/Nd：YAG double wave-length laser surgery is one of the safe and effect treatments for senior and high risk benign prostatic hyperplasia, with the advantages such as micro bleeding, short catheterization time, short bladder irrigation time and short hospitalization stay, which is worth of further promotion in clinical.
