摘要
目的探讨微创颅内血肿清除术治疗高血压性基底节区脑出血的临床疗效。方法选取2014年3月~2015年9月80例我院收治的高血压脑出血患者,对照组40例采用保守治疗,观察组40例实施微创颅内血肿清除术,对比两组NSE浓度和S-100蛋白浓度,比较各时段神经功能缺损评分、巴氏指数及临床疗效。结果观察组第7、14、28天的NSE水平与对照组比较差异有统计学意义(P〈0.05);观察组第14、28天的血清S-100蛋白浓度与对照组比较差异有统计学意义(P〈0.05);两组各时段神经功能缺损评分和BI评分的对比差异有统计学意义(P〈0.05);观察组总有效率为82.50%,对照组总有效率为57.50%,组间对比差异有统计学意义(P〈0.05)。结论微创颅内血肿清除术治疗高血压性基底节区脑出血疗效显著,值得在临床推广和运用。
Objective To investigate the clinical effect of microinvasive removal of intracranial hematoma in the treatment of hypertensive basal ganglia intracerebral haemorrhage. Methods 80 patients with hypertensive cerebral hemorrhage were selected,who were in our hospital from March 2014 to September 2015.40 cases of the control group were treated by conservative treatment.40 cases of observation group were given microinvasive removal of intracranial hematoma.The concentration of NSE and the concentration of S-100 protein of the two groups were compared.At different time periods,neurologic impairment score,Barthel index and clinical efficacy of two groups were compared. Results At the seventh day,the fourteenth day,and the twenty-eighth day,there was remarkable difference in the NSE level between the observation group and the control group(P〈0.05).At the fourteenth day and the twentyeighth day,there was remarkable difference in the serum S-100 protein concentration between the observation group and the control group(P〈0.05).In each period,the neurological impairment score and BI score of the two groups were significantly differents(P〈0.05).The total effective rate of observation group was 82.50%,and the total effective rate of control group was 57.50%.There was significant difference in the total effective rate between two groups(P〈0.05). Conclusion The curative effect of microinvasive removal of intracranial hematoma in the treatment of hypertensive basal ganglia intracerebral haemorrhage is remarkable.The microinvasive removal of intracranial hematoma in the treatment of hypertensive basal ganglia intracerebral haemorrhage is worth in clinical promotion and the application.
