原发性高血压中期肾损害中医证候学研究

The TCM Syndrome Study Of Primary Hypertensive Mid-Renal Damage

目的:通过对原发性高血压中期肾损害中医证候学的研究,得出原发性高血压中期肾损害的中医脏腑辨证病位和病性,用于指导临床辨证和治疗。方法:首先制定原发性高血压中期肾损害病例资料信息表,严格按照诊断标准收集病例资料,对62例原发性高血压中期肾损害患者进行中医证候积分分析,并以84例原发性高血压无肾损害者作为对照,进行中医证候脏腑积分统计学分析。结果:(1)原发性高血压中期肾损害组以肾脏、脾脏证候积分较高,且明显高于原发性高血压无肾损害患者组(P<0.01)。(2)原发性高血压中期肾损害组以气虚证、阴虚证证候积分较高,且明显高于原发性高血压无肾损害患者组(P<0.01)。(3)肾脏证候积分,气虚证、阴虚证,血瘀证证候积分在不同年龄段、不同级别血压下,积分呈上升趋势,大于70岁组及3级高血压组明显高于同类别其他组(P<0.01)。结论:(1)原发性高血压中期肾损害患者中医脏腑辨证病位为脾脏和肾脏,病性为气虚证和阴虚证。随着年龄的增长和高血压等级的增高,肾脏损害水平加重。气虚证、阴虚证程度加重。血瘀证贯穿于原发性高血压中期肾损害整个病程之中。

Objective： through the study of primary hypertension mid-renal damage and characteristics of TCM syndrome, To conclude differentiation of Zang and Fu of primary hypertension mid-renal damage, guide the further clinical syndrome differentiation and treatment. Method： first, formulate mid-renal damage information table, strictly collect clinical data in according to the criteria for the diagnosis, analyzing TCM syndrome integral of 62 patients with mid-renal damage, 84 patients without renal damage are contrasted with the 62.and then there are a statistical analysis of The TCM Syndrome among them. Results：（1） for the primary hypertension mid-renal damage group, the spleen and kidney syndrome integral is higher, the spleen and kidney syndrome integral significantly more than the primary hypertension without renal damage（P 0.01）.（2）for he primary hyper-tension mid-renal damage group,Qi deficiency syndrome and Yin deficiency syndrome integral is higher, and the Qi deficiency syndrome and Yin deficiency syndrome integral significantly more than the primary hypertension without renal damage（P 0.01）.（3） the kidney symptom integral under the different age groups and different levels of blood pressure, increasing integration, more than 70- year- old group and level 3 hypertension group, kidney integral was obviously more than the same category in the other groups（P 0.01）.（4） qi deficiency syndrome and Yin deficiency symptom integral under the different age groups, different levels of blood pressure, increasing integration, more than 70- year- old group and level 3 hypertension group, kidney integral was obviously more than the same category in the other groups（P 0.01）.（5） blood stasis symptom integral under the different age groups, different levels of blood pressure, increasing integration, among them, more than 70- year- old group and level 3 hypertension group, kidney integral was obviously more than the same category in the other groups（P 0.01）. Conclusion：（1） the primary hypertension midrenal damage of TCM with differentiation of Zang and Fu is spleen and kidney.（2） the primary hypertension mid-renal damage of TCM with differentiation of Zang and Fu of Yin is the qi deficiency syndrome and Yin deficiency syndrome.（3） increasing with age and blood pressure level, the kidney damage become more and more serious.（4） increasing with age and blood pressure level, qi deficiency syndrome and Yin deficiency syndrome become more and more serious.（5） blood stasis persist the whole of the primary hyperte-nsion mid-renal damage.
