补阳还五汤联合α-硫辛酸治疗气虚血瘀、脉络瘀阻型糖尿病周围神经病变临床疗效评价

Buyang Huanwu Decoction Combined with α-lipoic Acid in Treatment of Diabetic Peripheral Neuropathy Due to Qi Deficiency and Blood Stasis and Collateral Obstruction Type

目的:观察补阳还五汤联合α-硫辛酸治疗气虚血瘀型糖尿病周围神经病变的临床疗效。方法:将80例糖尿病周围神经病变患者随机分为治疗组和对照组各40例,两组均予以膳食指导,合理运动,降糖治疗,降压,降脂等基础治疗,对照组给予0.9%氯化钠注射250 m L、α-硫辛酸0.6静点,1次/d,治疗组在对照组的基础上加用中药复方补阳还五汤治疗,两组连续用药4周,观察糖尿病周围神经病变患者感觉及运动神经的传导速度、临床症状的变化及TCSS评分。结果:治疗组及对照组的临床症状在治疗前后均有显著性差异,且治疗组优于对照组(P<0.05);治疗组及对照组神经传导速度在治疗前后均有显著性差异,但治疗组与对照组比较无显著性差异(P>0.05)。结论:补阳还五汤联合α-硫辛酸能提高糖尿病周围神经病变患者感觉及运动神经的传导速度、显著改善患者肢体麻木疼痛的临床症状,降低TCSS评分,对糖尿病周围神经病变具有较好的疗效。

Objective： To observe the clinical efficacy of Buyang Huanwu Decoction combined with α- lipoic acid on Qi deficiency and blood stasis and collateral obstruction type of diabetic peripheral neuropathy. Methods： Eighty patients with diabetic peripheral neuropathy were randomly divided into treatment group and control group,40 cases in each. The two groups were both given the dietary guidance,reasonable exercise,hypoglycemic therapy,blood pressure- lowering and lipid- lowering therapy and other infrastructure. The control group was given 0. 9% chlorine sodium injection 250 m L,α- intravenous lipoic acid 0. 6,once a day. The treatment group on the basis of the control group used traditional Chinese medicine Buyang Huanwu Decoction. After continuous medication four weeks,observe diabetic peripheral neuropathy patients＇ sensory and motor nerve conduction velocity,changes in clinical symptoms and TCSS score. Results： The clinical symptoms in the treatment group and the control group were significant after treatment and the treatment group was better than the control group（ P〈0. 05）. Treatment and control groups＇ nerve conduction velocity was significantly different,but it showed no significant difference（ P〉0. 05）. Conclusion： Buyang Huanwu Decoction combined with α- lipoic acid can improve diabetic peripheral nerve sensory and motor nerve conduction velocity lesions,significantly improve numbness and pain and reduce TCSS score for diabetic peripheral neuropathy with good effect.
