慢性肝炎患者中医体质T淋巴细胞亚群表达的关系

Correlation Between Expressions of T Lymphocyte Subsets and TCM Constitutions of Chronic Hepatitis Patients

目的:探讨慢性肝炎患者中医体质与肝组织及外周血T淋巴细胞亚群表达的相关性研究。方法:将109例慢性肝炎患者作为研究对象,根据中医辨证理论将全部患者分为肝郁脾虚型、肝肾阴虚型、湿热中阻型、瘀血阻络型4种证型,分别检测患者外周血、肝组织CD4、CD8 T淋巴细胞亚群表达情况,分析中医证型与免疫表达的相关性。结果:肝肾阴虚型和瘀血阻络型患者外周CD4、CD8表达显著低于肝郁脾虚型和湿热中阻型患者,差异有统计学意义(P<0.05);4种证型患者外周血CD4/CD8比值差异无统计学意义(P>0.05);肝郁脾虚型患者肝组织CD4阴性表达及CD8阳性表达显著高于其余3种证型,差异有统计学意义(P<0.05);湿热中阻型和瘀血阻络型患者肝组织CD4、CD8强阳性表达显著高于肝郁脾虚型和肝肾阴虚型患者,差异有统计学意义(P<0.05)。结论:了解慢性肝炎患者外周血、肝组织CD4、CD8免疫表达情况,对中医辨证分析及临床治疗具有重要意义。

Objective： To investigate the correlation between TCM constitutions and expressions of T lymphocyte subsets of chronic hepatitis patients. Methods： A total of 109 cases of chronic hepatitis B patients were as research objects.According to the TCM theory,all the patients were divided into liver stagnation and spleen deficiency syndrome,liver-kidney Yin deficiency syndrome,damp- heat blockage syndrome and stasis blocking collateral syndrome. We detected the T lymphocyte subsets expressions and analyzed the correlation between TCM syndrome types and the expressions. Results：The liver- kidney Yin deficiency syndrome and stasis blocking collateral syndrome patients＇ CD4 and CD8 expressions in peripheral blood were significantly lower than those of liver stagnation and spleen deficiency syndrome and damp- heat blockage syndrome and the difference was statistically significant（ P〈0. 05）. Four kinds of syndromes＇ CD4/CD8 in peripheral blood had no significant difference（ P〈0. 05）. The CD4 negative expression and the positive expression of CD8 of patients with liver stagnation and spleen deficiency syndrome were significantly higher than those of the other three syndromes and the difference was statistically significant（ P〉0. 05）. The expressions of CD4 and CD8 in liver tissue of patients with damp- heat blockage syndrome and stasis blocking collateral syndrome were significantly higher than those of liver stagnation and spleen deficiency syndrome and Yin deficiency of liver and kidney patients and the difference was statistically significant（ P〈0. 05）. Conclusion： Understanding the expressions of CD4 and CD8 in peripheral blood and liver tissue of patients with chronic hepatitis has important significance for syndrome differentiation of traditional Chinese medicine and clinical treatment.
