不同针灸方法分期治疗贝尔面瘫的临床研究

Different Acupuncture Methods in Treatment of Bell's Facial Paralysis

目的:对比不同针灸方法分期治疗贝尔面瘫的临床疗效,为临床治疗方案的制定提供依据。方法:选取在医院就诊的贝尔面瘫患者288例,随机分为A组、B组和C组各96例。A组采用单纯针刺治疗,B组在针刺治疗的基础上,加用艾灸治疗,C组在针刺治疗的基础上,加用电针治疗。3组患者均接受20次治疗后,采用面部残疾指数(FDI)量表、面神经麻痹程度评分以及世界卫生组织生存质量测定量表简表(WHOQOL-BREF)评估患者的临床治疗效果。结果:3组患者治疗前、治疗后FDI量表、面神经麻痹程度评分以及WHOQOL-BREF量表评分结果的组间对比,差异均没有统计学意义(均P>0.05);3组患者治疗后的躯体功能总分均显著高于治疗前,而社会功能总分显著低于治疗前(均P<0.05);3组患者治疗前后的WHOQOL-BREF量表评分结果的组间对比,差异均没有统计学意义(均P>0.05);3组患者治疗后的面神经麻痹程度评分均显著高于治疗前(均P<0.05)。结论:单纯针刺、针刺配合艾灸、针刺配合电针治疗贝尔面瘫均可显著改善贝尔面瘫患者的临床症状,且3种疗法的临床疗效相当,临床可根据自身的医疗条件选择适宜的治疗方案。

Objective： To compare the clinical effect of different acupuncture methods in the treatment of Bell＇＇s facial paralysis and provide the basis for the clinical treatment plan. Methods： A total of 288 patients with facial paralysis treated in our hospital were randomly divided into A group,B group and C group,96 cases in each. The A group was treated with simple acupuncture,the B group treated with acupuncture and moxibustion and the C group treated with eletroacupuncture. Three groups were treated for 20 times. The use of facial disability index（ FDI） scale,facial nerve paralysis score and the World Health Organization Quality of life（ WHOQOL- BREF） assessment of patients were measured for evaluating the clinical treatment effect. Results： Before treatment,FDI scale,facial nerve paralysis score and WHOQOL-BREF score of three groups were not statistically significant. After treatment,the total scores of three groups were significantly higher than and social function scores were significantly lower（ P〈0. 05）. The difference was not statistically significant（ P = 0. 05）. Three groups＇ facial nerve palsy were significantly higher than those before treatment（ P〈0. 05）.Conclusion： Simple acupuncture,acupuncture combined with moxibustion and acupuncture combined with electroacupuncture treatment of Bell＇s facial paralysis can significantly improve the clinical symptoms of and the clinical efficacy of the three therapies is quite different. The appropriate treatment should be chosen according to their own medical conditions.
