黄芩联合湿润烧伤膏治疗老年慢性顽固性皮肤溃疡的临床疗效观察

Clinical Observation of Combination Use of Scutellaria Baicalensis Georgi and Moist Exposed Burn Ointment(MEBO)in the Treatment of Chronic Intractable Skin Ulcer in Aged Patients

目的探讨黄芩联合湿润烧伤膏(MEBO)治疗老年慢性顽固性皮肤溃疡的临床疗效。方法选取襄阳市某医院2012年11月-2014年11月收治的60例老年慢性顽固性皮肤溃疡患者作为研究对象,随机分为对照组和观察组,每组30例。对照组患者采用MEBO治疗,观察组患者在对照组的基础上给予黄芩汁治疗,比较2组患者的临床疗效、腐肉净化时间、肌生时间、愈合时间,同时在治疗前后采用主要症状和健康调查简表(SF-36)对患者进行评分。结果观察组患者的治疗总有效率为96.67%,明显高于对照组(73.33%),差异具有统计学意义(P<0.05)。治疗后,观察组患者的腐肉净化时间、肌生时间、愈合时间分别为(12.05±3.54)d,(36.37±5.09)d,(51.06±12.85)d,均短于对照组,差异有统计学意义(P<0.05);观察组患者的红肿、疼痛、腐肉、脓性分泌物评分分别为0.59±0.53,0.62±0.35,0.38±0.53,0.44±0.41,均明显低于对照组,差异有统计学意义(P<0.05)。治疗15d,30d时观察组患者SF-36评分分别为85.06±11.35,92.69±14.21,均明显高于对照组,差异具有统计学意义(P<0.05)。结论黄芩联合MEBO治疗老年慢性顽固性皮肤溃疡,能够有效改善创面红肿、疼痛症状,缩短肉芽组织生长、创面愈合时间,临床疗效显著,值得推广。

Objective To discuss the clinical effect of combination use of scutellaria baicalensis georgi and Moist Exposed Burn Ointment（MEBO）in the treatment of chronic intractable skin ulcer in aged patients.Methods The study was carried on by selecting 60 old patients with chronic intractable skin ulcer from Nov.2012 to Nov.2014 in one of Xiangyang hospital.Patients were randomly divided into control group and observation group with 30 cases in each one,and they adopted therapies of MEBO and combination use of scutellaria baicalensis georgi and MEBO separately.Curative effect,recovery time of slough,muscle growth time and healing time was compared between two groups,symptoms before and after treatment were assessed,and Shot Form 36 Health Survey Questionnaire（SF-36）was employed.Results The total effective rate of observation group was 96.67%,which was higher than that of control group（73.33%）.This difference was significant（P〈0.05）.Recovery time of slough,muscle growth time and healing time of observation group was（12.05±3.54）d,（36.37±5.09）d and（51.06±12.85）d respectively,which was shorter than that of control group with significant difference（P〈0.05）.The assessment scores of patients in observation group after treatment with red and swollen,pain,slough and purulent secretion were 0.59±0.53,0.62±0.35,0.38±0.53,0.44±0.41,and they were all higher than those of control group.The differences were statistically significant（P〈0.05）.after 15 dand 30dof treatment,the scores of SF-36 in the observation group were 85.06±11.35and92.69±14.21 respectively which were significantly higher than those in the control group（P〈0.05）.Conclusion Combination use of scutellaria baicalensis georgi and MEBO in the treatment of chronic intractable skin ulcer in aged patients can relieve swollen and pain,shorten grow time of granulation,and shorten healing time.The curative effect is significant,so the treatment deserves to be promoted.
