米索前列醇、缩宫素联合钙剂防治高危孕妇剖宫产后出血的临床效果

Clinical effect of misoprostol and oxytocin combined with calcium gluconate on the prevention and treatment of postpartum hemorrhage in high-risk pregnant women with cesarean section

目的探讨米索前列醇、缩宫素联合钙剂防治高危孕妇剖宫产后出血的临床效果。方法 120例有高危出血的剖宫产产妇随机分为实验组和对照组,对照组仅做缩宫素处理,实验组术前给予葡萄糖酸钙以及肛塞米索前列醇处理,术后再以米索前列醇置于直肠处理。比较2组患者的临床治疗效果。结果实验组患者第三产程、术中出血量、术后2 h出血量、术后24 h出血量以及产后出血发生率均显著优于对照组(P<0.05)。2组治疗后血压、心率、呼吸频率及血氧饱和度无显著差异(P>0.05)。2组不良反应发生率及新生儿1 min Apgar评分无显著差异(P>0.05)。结论米索前列醇、缩宫素联合钙剂对防治高危产妇剖宫产后出血效果较佳,临床上使用安全有效。

Objective To explore the clinical effect of misoprostol and oxytocin combined with calcium gluconate on the prevention and treatment of postpartum hemorrhage in high-risk pregnant women with cesarean section. Methods A total of 120 patients with high-risk bleeding were randomly divided into experimental group and control group. The control group was treated with oxytocin,and the experimental group was treated with calcium gluconate and misoprostol on the basis of the control group. After operation,misoprostol（ 0. 2 mg） was placed in the rectum. Clinical effect was compared between two groups. Results Patients in the experimental group were significantly better than the control group in the third stage,the amount of bleeding,2 h bleeding volume,24 h bleeding volume and the incidence rate of postpartum hemorrhage（ P〈 0. 05）. There were no significant differences in blood pressure,heart rate,respiratory rate and blood pressure between two groups（ P〉 0. 05）. There were no significant differences in the incidence rate of adverse reactions and score of neonatal 1 min Apgar（ P〉 0. 05）. Conclusion The effect of misoprostol and oxytocin combined with calcium gluconate in the prevention and treatment of postpartum hemorrhage of high-risk pregnant women with cesarean section is good,and it is safe and effective.
