超声乳化联合房角分离术治疗闭角型青光眼合并白内障疗效分析

Clinical analysis of treating angle-closure glaucoma complicated with senile cataract with phacoemulsification combined with goniosynechialysis

目的 观察和分析超声乳化人工晶状体植入术联合房角分离术治疗闭角型青光眼合并白内障的疗效.方法 对2012年12月至2014年12月在四平市中心医院眼科诊治的82例（86只眼）闭角型青光眼合并不同程度白内障患者行超声乳化人工晶状体植入术联合房角分离术治疗.术后随访6个月,观察术后视力、眼压、中央前房深度、房角形态变化、视野和不良反应发生情况.结果 86只眼术前经降眼压治疗后眼压控制在（20.3±7.8） mmHg,术后6个月测眼压为（14.7±5.2） mmHg,差异有统计学意义（P＜0.05）.中央前房深度由术前（1.624±0.258） mm增加到术后（3.214±0.174） mm,差异有统计学意义（P＜0.05）.84只眼术后视力明显提高.术后房角形态明显改善,视野无缩小,视觉敏感度增强,术中、术后无严重手术并发症.结论 超声乳化联合房角分离术治疗闭角型青光眼合并白内障的疗效显著,安全性高.

Objective To observe the effects and morphological changes of angle treating angle-closure glaucoma complicated with senile cataract with phacoemulsification combined with goniosyneihialysis.Methods Eighty-two patients （86 eyes） suffered from acute or chronic angle-closure glaucoma complicated with varying degrees senile cataract were selected.The eyesight,intraocular pressure,vision,depth of central anterior chamber and angle morphology of all eyes were compared preoperative and postoperative.Results All patients were followed up for 6 months.The intraocular pressure of 86 eyes was obviously decreased after operation.The intraocular pressures of preoperative and postoperative were （20.3±7.8） and （14.7±5.2） mrnHg,respectively.The depth of anterior chamber of 86 eyes was deepened,the depth preoperative and postoperative were （1.624±0.258） and （3.214±0.174）mm,respectively.The eyesight of 84 eyes were obviously improved after operation,the shape of the angle on post-operation was obviously improved.No narrow field of vision,increased significantly compared with the preoperative visual acuity.There were no serious postoperative complications.Conclusions Phacoemulsification combined with goniosynechialysis is an effective method to treat angle-closure glaucoma complicated with senile cataract,raising eyesight and comfort.
