甲状腺癌多普勒血流动力学指标与肿瘤增殖、血管新生指标的相关性分析 被引量:27

摘要 目的:研究甲状腺癌多普勒血流动力学指标与肿瘤增殖、血管新生指标的相关性。方法:108例甲状腺癌患者经B超及病理学确诊后纳入观察组,另取同期在本院进行甲状腺腺瘤切除术的非癌患者107例作为健康对照组,检测两组甲状腺血流动力学指标、肿瘤增殖相关指标、血清血管新生相关指标,进一步分析甲状腺癌血流动力学指标与肿瘤增殖、血管新生指标的相关性。结果:观察组血流频谱收缩期峰速(S)、舒张末期血流速度(D)值高于对照组(P<0.05),组间收缩期/舒张期流速(S/D)、阻力指数(RI)、搏动指数(PI)值差异无统计学意义(P>0.05);观察组甲状腺肿瘤p53、增殖细胞核抗原(PCNA)、Ki-67表达量高于对照组,肿瘤坏死因子诱导蛋白8样蛋白2(TIPE2)表达量低于对照组(P<0.05);观察组血清血管内皮生长因子(VEGF)、血管生成素-2(Ang-2)、缺氧诱导因子-1α(HIF-1α)、胰岛素样生长因子-Ⅱ(IGF-Ⅱ)、内皮抑素(endostatin)值高于对照组,甘露糖结合凝集素(MBP)值低于对照组(P<0.05);S、D与p53、PCNA、Ki-67、VEGF、Ang-2、HIF-1α、IGF-Ⅱ、endostatin值呈正比,与TIPE2、MBP值呈反比(P<0.05)。结论:甲状腺癌患者动脉血流流速与肿瘤增殖及血管新生直接相关,可以作为判断肿瘤病情及治疗效果的可靠指标。 Objective:To discover the correlation of thyroid cancer Doppler hemodynamic indexes with tumor proliferation and angiogenesis indexes.Methods:A total of 108 thyroid cancer patients were diagnosed by B-ultrasound and pathology and then included in the observation group of the research,while 107 cases of non-cancer patients who received excision of thyroid adenoma in our hospital during the same period were selected as healthy control group.Thyroid hemodynamic indexes,tumor proliferation-related indexes and serum angiogenesis-related indexes of two groups were detected,and the correlation of thyroid cancer hemodynamic indexes with tumor proliferation and angiogenesis indexes was further analyzed.Results:thyroid artery peak systolic velocity(S)and end diastolic velocity(D)values of observation group were significantly higher than those of control group(P〈0.05),and there were no significant differences in S/D,resistance index(RI)and pulsatility index(PI)values between groups(P〉0.05);p53,proliferating cell nuclear antigen(PCNA)and Ki-67 expression levels in thyroid tumor of observation group were significantly higher than those of control group while tumor necrosis factor-αinduced protein-8-like 2(TIPE2)protein expression level was significantly lower than that of control group(P〈0.05);serum vascular endothelial growth factor(VEGF),angiogenin-2(Ang-2),hypoxia induction factor-1α(HIF-1α),insulin-like growth factor-Ⅱ(IGF-Ⅱ)and endostatin values of observation group were significantly higher than those of control group while MBP value was significantly lower than that of control group(P〈0.05);S and D were directly proportional to p53,PCNA,Ki-67,VEGF,Ang-2,HIF-1α,IGF-Ⅱ and endostatin values,and inversely proportional to TIPE2 and mannma binding protein(MBP)values(P〈0.05).Conclusions:Artery blood flow velocity in patients with thyroid cancer is directly correlated with tumor proliferation and angiogenesis,and can be used as the reliable index to judge tumor condition and curative effect.
