以临床路径为基础的终末期肾病患者心理问题分析与护理对策

Analysis on Nursing Countermeasures and Psychological Problems of the Patients with End-stage Renal Disease Based on Clinical Pathway

目的:分析以临床路径为基础的终末期肾病(ESRD)患者的心理问题,并提出护理对策。方法:将90例患者随机分为观察组和对照组各45例。对照组仅进行常规护理,观察组在对照组常规护理的基础上进行心理护理干预。入院第1天及出院前1天,均采用症状自评量表(SCL-90)对患者心理健康状况进行测试评分,比较2组患者治疗前后心理健康状况改善情况。结果:抑郁、人际关系、焦虑、恐惧、敌对等因子分干预后2组比较,差异有统计学意义(P<0.05);抑郁、人际关系、焦虑、恐惧、敌对等因子分值观察组干预前后比较,差异有统计学意义(P<0.05)。结论:对ESRD患者进行治疗的同时做好心理护理,对改善愈后具有重要的临床意义。

Objective： To raise psychological problems of the patients with end-stage renal disease （ESRD） based on clinical pathway, and raise nursing countermeasures. Methods： Ninety patients were randomized into the observation group and the control group, 45 cases each group. The control grop received routine care, the observa- tion group accepted psychological nursing intervention on the foundation of routine care the control group received. Psycological healthy conditions of the patients were tested and scored by adopting SCL-90 at the first day of admis- sion day and the last day before discharged, psychological health of the patients in both groups were compared be- tween before and after treating. Results： The difference had statistical meaning in the comparison of the score of the factors including depression, social relationship, anxiety, fear, hostility between both groups after intervention （P〈 0.05）; the difference showed statistical meaning in the comparison of the score of the factors including depression, social relationship, anxiety, fear, hostility between before and after intervention in the observation group （P〈0.05）. Conclusion： Psychological nursing intervention given to ESRD patients who received the treatment simultaneously show important clinical meaning to improve the prognosis.
