摘要
目的评估中国医院侵袭性真菌监测网(CHIF-NET)10和11收集的近平滑念珠菌复合体原始鉴定、Bruker MS与VITEK MS鉴定结果。方法经中心实验室分子生物学方法复核鉴定确认,CHIF-NET 10和11从全国22所医院共收集到近平滑念珠菌复合体352株,包括近平滑念珠菌305株,似平滑念珠菌36株,拟平滑念珠菌11株;以复核鉴定结果为金标准,评价各单位原始常规方法鉴定结果,并评估Bruker MS和VITEK MS基质辅助激光解吸电离飞行时间质谱(MALDI-TOF MS)对近平滑念珠菌复合体的鉴定性能。结果与分子生物学鉴定结果相比,近平滑念珠菌的各医院原始鉴定准确率为90.8%,9.2%的菌株被API 20C或VITEK-2错误鉴定为光滑念珠菌、季也蒙念珠菌等;常规方法无法鉴定似平滑念珠菌和拟平滑念珠菌;Bruker MS和VITEK MS对305株近平滑念珠菌鉴定准确率分别达到了98.4%和97.7%;Bruker Biotyper 3.0数据库中有似平滑念珠菌和拟平滑念珠菌参考图谱,但鉴定准确率仅分别为33.3%和27.3%;VITEK MS V2.0没有两种菌的参考图谱而无法鉴定。结论该项研究率先在国内评估了Bruker MS和VITEK MS鉴定近平滑念珠菌复合体的性能,证明二者对于近平滑念珠菌的鉴定是比较可靠的。
OBJECTIVE To evaluate the results of original identification,Bruker MS,and VITEK MS identification of Candida parapsilosis complex collected from National China Hospital Invasive Fungal Surveillance Net(CHIFNET)10and 11.METHODS A total of 352 isolates of C.parapsilosis that were confirmed by molecular identification were collected from 22 hospitals in China by CHIF-NET 10 and 11,including 305 C.parapsilosisisolates,36 Candida metapsilosis isolates,and 11 Candida orthopsilosis isolates.The original identification results were evaluated by setting the review identification results as golden standards,and the performances of Bruker MS and VITEK MS matrix assisted laser desorption ionization time of flight mass spectrometry(MALDI-TOF MS)in identification of the C.parapsilosis complex were observed.RESULTS As compared with the molecular identification,the accurate rate of original identification of C.parapsilosis was 90.8%in the hospitalsm,and 9.2% of the strains were mistakenly identified as C.glabrata and Pichia guilliermondii;C.metapsilosis and C.orthopsilosis could not be identified by using routine identification methods.As for the identification of the 305 C.parapsilosisisolates,the accurate rates of the Bruker MS and VITEK MS were 98.4% and 97.7%,respectively.There were reference spectrums for C.metapsilosis and C.orthopsilosisin the Bruker Biotyper 3.0database,however,the accurate rates of identification were only 33.3% and 27.3%,respectively;the two species could not be identified owing to no reference spectrums in VITEK MS V2.0.CONCLUSIONIt is for the first time in China to evaluate the performances of Bruker MS and VITEK MS in identification of the C.parapsilosis complex and is concluded that the two methods are reliable for the identification of the C.parapsilosis.
