超声造影诊断乳腺导管内乳头状瘤的研究

The Research of Ultrasound Contrast in the Diagnostic of Intraductal Papillomas

目的探讨超声造影对乳腺导管内乳头状瘤的诊断价值。方法选择乳腺导管内乳头状瘤患者80例,其中恶性肿瘤患者（恶性组）20例,良性肿瘤患者（良性组）60例,两组均进行常规超声与超声造影检查。结果常规超声显示恶性组多表现为乳腺腺体致密、微小钙化、边界缘毛刺和肿块影（P〈0.05）;良性组的超声弹性成像评分多为2~3分,平均为（2.56±0.44）分;而恶性组多为4~5分,平均分为（4.56±0.32）分,两组对比差异有统计学意义（P〈0.05）。在恶性组20例患者中,乳腺导管内乳头状瘤的超声造影评分与其Dukes分期、分化程度与淋巴结转移显著相关（P〈0.05）。结论超声造影诊断乳腺导管内乳头状瘤能有很好的影像学特征,能有效对良恶性状况进行鉴别诊断,并且与病理分化存在明显的相关性,为乳腺癌的早期诊断提供有意义的参考价值。

Objective To evaluate the diagnostic value of ultrasound contrast for intraductal papillomas. Methods 80 cases of intraductal papillomas including 20 patients with malignant tumors（ the malignant group） and 60 patients with benign tumors（ the benign group） were selected,both groups were given the conventional ultrasound and ultrasound imaging. Results Compared with the benign group,ultrasound for the malignant group showed dense microcalcifications,boundary edge burrs and mass movies（ P〈0. 05）; The ultrasound elastography scores in the benign group were mainly 2 ~ 3 scores,the average values was（ 2. 56 ± 0. 44） points; while the malignant group were mainly 4 ~ 5 scores,and the average values was（ 4. 56 ± 0. 32） points,the difference was statistically significant（ P〈0. 05）. In the malignant group,intraductal papilloma ultrasound contrast scores were significantly related with Dukes stage,degree of differentiation and lymph node metastasis tissue（ P〈0. 05）. Conclusion Ultrasound contrast imaging in the diagnosis of breast intraductal papillomas has good imaging features,it is effective in the differential diagnosis of benign and malignant status,and is significanty correlated with histological differentiation,and it can provide meaningful reference for the early diagnosis of breast cancer.
