基层医院丙型肝炎筛查管理的探索与实践

Exploration and practice of screening management of hepatitis C in primary hospital

目的探讨基层医院实施丙型肝炎筛查管理的有效措施。方法以问卷形式随机调查我院医生(非肝病专科医生)70名,护士30名,随机调查我院门诊患者100名(与所患疾病种类无关),分别于教育前(2011年)及教育后(2014年)完成。结果教育前后认识高危性行为是丙肝高危因素的医护组人数为50名和83名(P<0.05),患者组为36名和61名(P<0.05);教育前后认识纹身、穿孔、针灸为其高危因素的医护组人数为39名和67名(P<0.05),患者组为28名和49名(P<0.05);教育前后知晓专科就诊的医护组人数为39名和67名(P<0.05),患者组为28名和48名(P<0.05);教育前后传染病漏报率为29名和1名(P<0.05)。结论在基层医院丙肝的筛查管理中健康教育是有效措施,行政管理必不可少。

Objective Discussion effective measures and standard management for implement of hepatitis C screening in primary hospital. Methods A random survey was completed among 70 doctors （non-liver disease specialist） and 30 nurses, before the education （the year of 2011 ） and after the post-secondary education （the year of 2014） with the same questionnaire. Also 100 patients in our hospital （regardless of the type of illness） were included in the same random survey. The questionnaire for doctors and patients were the same. Results Before and after education： the number of doctors who understanding the high-risk sexual behavior is the medical risk factors for hepatitis C were 50/100 and 83/100 （P〈0.05）,patients were 36/100 and 61/100 （P〈0.05）; Before and after education：the number of doctors who recognize that tattoos, piercing, acupuncture may induce people infected with HCV were 39/100 and 67/100（P〈0.05 ）, the patients were 28/100 and 49/100（P〈0.05 ） ; Before and after education： the number of doctors who awareness of the need for specialist visits were 39/100 and 67/100 （P〈0.05） ;the patients were 28/100 and 48/100 （P〈0.05） .Before and after education：the under-report rate of infectious diseases were 29/40 and 1/50 （P〈0.05）. Conclusion A scientific health education is an effective measure for the standardized management of screening of hepatitis C in local hospital, also administration management is essential.
