不孕症患者心理状态及心率变异性的调查分析

Investigation and Analysis of Psychological Status and Heart Rate Variability in Patient with Infertility

目的:探讨不孕症患者的心理状态及心率变异性,以此评估不孕症患者的自主神经功能,从而为不孕症患者的心理治疗提供理论依据。方法:选取2015年1月至2015年6月就诊于重庆医科大学附属第二医院生殖中心的不孕症患者120例作为观察组,选取同期就诊进行孕前检查的128例正常育龄期女性作为对照组。采用焦虑自评量表及抑郁自评量表调查两组的心理状态,用心率变异性分析仪检测两组的心率变异性各指标。结果:观察组焦虑31例(占25.83%),抑郁49例(占40.83%),对照组焦虑12例(占9.37%),抑郁26例(占20.31%)。两组的焦虑发生率及抑郁发生率比较差异均有统计学意义(P<0.05)。观察组的焦虑评分(44.93±8.94分)及抑郁评分(49.02±11.23分),显著高于对照组的焦虑评分(41.00±8.24分)及抑郁评分(42.77±10.38分)(P<0.05)。观察组心率变异性检测中的正常心动周期的标准偏差(SDNN)、总能量(TP)、低频功率(LF)、高频功率(HF)明显低于对照组(P<0.05)。两组的LF/HF比较差异无统计学意义(P>0.05)。结论:不孕症患者较易出现焦虑、抑郁等不良情绪,心率变异性降低,自主神经系统活性减弱。针对不孕症患者的治疗,我们应当重视心理治疗。

Objective：To study the psychological status and heart rate variability of infertility patients and eval- uate their autonomic nervous function, as well as providing a theoretical basis for the treatment of infertility pa- tients. Methods：We choose 120 patients as the observation group from visiting The Reproductive Medical Center of the Second Affiliated Hospital of Chongqing Medical University from January 2015 to June 2015,and choose 128 females of childbearing age coming for preconception checkups as the control group in the same time. Self-rating Anxiety Scale and Self-rating Depression Scales were used to investigate the psychological state of the two groups. The indexes of the heart rate variability of the two groups were detected by the method of heart rate variability analyzer. Results： In the observation group, there were 31 cases （25.83%） of anxiety, depressive mood in 49 cases （40.83%）, while in the control group, there were 12 cases （9. 37% ） of anxiety, depressive mood in 26 cases（20.31%）. There were significant differences in the incidence of anxiety and depression be- tween the two groups（ P 〈 0.05）. In the observation group,the anxiety and depression was more severe. Anxiety score and depression score in the control group were （41.00 ± 8.24） and（42.77 ± 10.38）, respectively. In the observation group they were （44.93 ± 8.94） and （49.02 ± 11.23）, which was significant higher （ P 〈 0.05）. The SDNN/TP/LF/HF of heart rate variability in the observation group were significantly decreased compared with the control group（P〈0.05）. There were no significant statistical differences oi LF/HF in the two groups（ P〉0. 05）. Conclusions：Infertility patients are prone to anxiety,depression and other negative emotions, and as infertility pa- tients＇ heart rate variability is reduced, they might have autonomic nervous system dysfunction. Thus, for a better treatment of infertility we should pay attention to the psychological treatment.
