显微手术夹闭和血管内介入治疗大脑中动脉动脉瘤的对比研究

A comparative study of microsurgical clipping and endovascular intervention in the treatment of middle cerebral artery aneurysms

目的探讨显微手术夹闭和血管内介入治疗大脑中动脉动脉瘤的对比疗效。方法选取2011年10月-2013年12月我院收治的颅内动脉瘤患者115例,所有患者随机分为观察组58例及对照组57例。观察两组患者手术前后Hunt-Hess分级、术后并发症情况及住院情况（包括住院时间、医疗费用）。结果两组患者手术前后Hunt-Hess分级比较,差异不显著（P〉0.05）。观察组患者的并发症发生率为13.79%,明显低于对照组的36.84%,两组比较,差异显著（P〈0.05）。观察组患者住院时间短于对照组,住院费用高于对照组,两组比较,差异显著（P〈0.05）。结论血管内介入动脉瘤栓塞术具有创伤小、治疗时间短、术后生存质量高的特点,但其费用较高,所以临床上应根据患者具体情况选择合适的术式。

Objective To explore the microsurgical clipping and endovascular intervention in the treatment of middle cerebral artery aneurysms. Methods From October 2011 to December 2013, a total of 115 patients with intracranial aneurysms were collected and were randomly divided into observation group（n=58） and control group（n=57）. Two groups of patients before and after the operation of Hunt-Hess classification, postoperative complications and hospitalization（including hospital stay, medical expenses） were all observed and compared. Results Comparison of Hunt Hess classification of two groups of patients before and after surgery, the difference was not significant（P〉0.05）. The complication rate of observation group was 13.79%, which was significantly lower than that in control group（36.84%）, the difference was significant（P〈0.05）. The hospitalization time of observation group patients was shorter than the control group, the cost of hospitalization was higher than the control group, the differences wwew significant between the two groups（P〈0.05）. Conclusion Transcatheter arterial embolization has the characteristics of small trauma, short treatment time and high quality of life, but the cost is high, so it should be chosen according to the specific circumstances of the patients.
