柴胡加龙骨牡蛎汤加减方治疗乳腺癌并发抑郁患者的临床研究

CLINICAL STUDY OF CHAIHU PLUS LONGGUMULI DECOCTION IN THE TREATMENT OF DEPRESSION IN PATIENTS WITH BREAST CANCER

目的评价柴胡加龙骨牡蛎汤加减方对改善乳腺癌患者抑郁情绪的有效性。方法 2013年1月—2016年1月唐山市人民医院收治乳腺癌并发抑郁患者298例,随机分为观察组(柴胡加龙骨牡蛎汤加减方治疗)148例和对照组150例(氟西汀治疗),治疗30d。治疗前后使用汉密尔顿抑郁量表(HAMD)和生活质量综合评定量表(QLQ-C30)进行测量。结果 1柴胡加龙骨牡蛎汤加减方可以改善患者的临床症状,降低HAMD分值,优于对照组,差异有统计学意义(P<0.05)。2二组患者在角色功能、认知功能、情绪功能、总体生活质量、疲乏、恶心呕吐、疼痛、睡眠障碍、食欲丧失、便秘、腹泻方面比较差异有统计学意义(P<0.05)。3观察组不良反应发生率为20.9%(31/148),显著低于对照组36.0%(54/150),差异有统计学意义(P<0.05)。结论柴胡加龙骨牡蛎汤加减方对乳腺癌并发抑郁患者的治疗安全有效,较氟西汀治疗更有利于乳腺癌患者抑郁情绪和生活质量的改善。

Objective To evaluate the effectiveness of Chaihu plus Longgumuli Decoction for depression in patients with breast cancer. Methods In this randomized controlled trial,298 breast cancer patients compli- cated with depression were randomly divided into the treatment group and control group. The 148 subjects in the treatment group were treated with Chaihu plus Longgumuli Decoction and the 150 subjects in the control group were treated with Fluoxetine. All patients were assessed by Hamilton depression rating scale {HAMD） and the quality of life rating scale （QLQ C30） measurements before and 30 days after treatment. Results The Chaihu plus Longgumuli Decoction significantly improved the patient＇s clinical symptoms and reduced the HAMD scores compared with fluoxetine （P〈0.05）. The Chaihu plus Longgumuli Decoction significantly improved the quality of life of patients compared with fluoxetine （ P〈0.05）. The incidence of adverse reactions in the treatment group （26.5%） was significantly lower than the control group （55.1%） （P〈0. 05）. Conclusions The Chaihu plus Longgumuli Decoction for the treatment of patients with breast cancer complicated with depression is safe and effective for the treatment of depression in patients with breast cancer and is better in the improvement of the quality of life than Fluoxetine.
