循证护理在高位复杂性肛门直肠瘘手术治疗恢复中的应用

APPLICATION OF EVIDENCE-BASED CARE IN HIGH COMPLEX ANORECTAL FISTULA SURGERY RECOVERY

目的分析与探讨高位复杂性肛门直肠瘘手术治疗恢复中应用循证护理的临床价值。方法针对该医院2013年12月—2015年11月收治116例高位复杂性肛瘘患者,依据随机数字表的方法,将其分为观察组(58例)和对照组(58例),对所有患者行Ligasure(TM)手术治疗,术后对对照组患者行常规护理干预,在对照组护理基础上,对观察组患者行循证护理干预,观察与对比二组患者护理与治疗后的各项指标。结果观察组和对照组患者角色功能、总体健康等生活质量评分差异无统计学意义(P>0.05),观察组患者情感功能评分明显低于对照组(P<0.05),术后观察组患者并发症发生率3.4%,对照组为31.0%,二组并发症发生率差异有统计学意义(P<0.05),改善患者术后疼痛方面,对照组患者与观察组疼痛改善有效率分别为72.4%、95.6%,观察组明显优于对照组,具有统计学意义(P<0.05)。结论循证护理干预应用于高位复杂性肛门直肠瘘手术中,有助于提升患者生活质量,同时改善患者术后疼痛现象,降低术后并发症发生率,值得临床应用与推广。

Objective To analyze and investigate the clinical value of high complex anorectal fistula surgery recovery EBN. Methods For the hospital in December 2013 and November 2015 admitted 116 cases of pa- tients with high anal fistula complexity, according to the random number table method, they were divided into the study group （58 cases） and the control group （58 cases）. Ligasure （TM） surgical interventions was for all patients, the control group underwent routine nursing intervention, based on the control group, study group patients were with evidence - based nursing interventions, then the observation and comparison of two groups of patients were performed on index after treatment. Results The study group and the con- trol group in patients with functional roles, overall health, and quality of life score were not significantly different, P〉0. 05. The study group patients emotional function score was significantly lower than the control group, P〈0.05. After surgery, the study group was 3.4% complication rate, while the control group was 31.0%, the deference of complication rate between the two groups was of relatively significant, P〈0.05, statistically significant improvement in patients with postoperative pain aspect, the control group of patients with pain and improve efficiency of the study group was 72.4% and 95.6% in the study group and the control group, P〈0. 05. Conclusions Evidence-based nursing intervention applies to high complex anorectal fistula surgery, helps to improve the quality of life of patients, while improves postoperative pain phenomenon, reduces the incidence of postoperative complications, it is worthy of clinical application and promotion.
