三阴性乳腺癌的临床病理特征和EGFR、Ki67的表达及相关性

目的比较TNBC与non-TNBC在临床病理特征及EGFR、Ki67表达的差异,探讨EGFR与Ki67表达的相关性。方法收集经病理确诊的乳腺癌患者276例,包括TNBC 42例(15.6%),non-TNBC 234例(84.4%),比较两者的临床病理特征的异同。采用免疫组化法检测EGFR和Ki67在两者间的表达差异及分析两者的相关性。结果两组在发病年龄、肿块大小、组织学分级的差异无统计学意义(P>0.05),但TNBC组淋巴结转移比例高(61.9%比43.2%,P<0.05);EGFR和Ki67在TNBC组中的阳性表达率均明显高于non-TNBC组(83.3%,85.7%比58.1%,64.1%,P<0.05),EGFR与Ki67仅在TNBC组中的表达有相关性(r=0.365,P=0.017),non-TNBC组中两者无相关性(r=0.069,P=0.293)。结论 TNBC较non-TNBC更易发生淋巴结转移,EGFR与Ki67在TNBC中阳性表达率高于non-TNBC且两者表达有相关性,可能与TNBC不良预后相关,可望为TNBC的治疗寻找新的治疗靶点。

Objective To compare the differences in clinicopathologic features and the expression of EGFR,Ki67 between TNBC and non-TNBC ,then to explore the correlation between Ki67 and EGFR expression. Methods We selected 276 patients with histopathologically confirmed breast cancer,including 42 cases of triple negative breast cancer （15.6%）,non-triple negative breast cancer 234 eases（84.4%）. We analyzed the difference in clinicopathologic features, Immunohistochemistry was used to detect the expression of EGFR and Ki67, and then investigated the difference and the correlation between them. Results The two groups was not statistically significant in age,tumor size ,histological grade difference （/9〉0.05）,But the proportion of highly metastatic lymph nodes in TNBC （61.9%VS 43. 2% ,P〈0.05.）;EGFR and Ki67 expression rates in TNBC group was significantly higher than non-TNBC group（83.3% ,85.7% vs 58.1% ,64.1% ,P〈0.05）,The expression of EGFR was correlated with Ki67 in TNBC （r=0.365, P〈0.05）, but the expression of EGFR was not correlated with Ki67 in non-TNBC（r=-0.069,P〉0.05）. Conclusion TNBC shows high- er lymph node metastasis with non-TNBC, EGFR and Ki67 showed high expression in TNBC and they were eorrlated, which indi- cates that they may be related to poor prognosis of TNBC. And can provide new therapeutic targets for TNBC.
