新基因MTB139在儿童结核临床分离株中的表达及其临床意义的初步研究

摘要 目的:检测并验证新基因MTB139在儿童结核临床分离株中的表达,通过比较其在不同耐药性结核结核分枝杆菌中的表达差异,探讨其与结核耐药性的关系。方法:应用逆转录聚合酶链式反应(reverse transcription polymerase chain reaction,RT-PCR)的方法检测60例临床结核分离株(9例耐多药株,24例非耐多药株,27例敏感株)中基因MTB139的表达。结果:60例结核分离株样本中有7例检测到MTB139基因的阳性表达,3个组(耐多药组、非耐多药组、敏感组)结核菌株中MTB139基因阳性表达率比较有统计学差异(P=0.000),其中耐多药组MTB139基因阳性表达率(5例,阳性表达率55.56%)明显高于非耐多药组(1例,阳性表达率4.17%)及敏感组(1例,阳性表达率3.70%),差异有统计学意义(P=0.003、P=0.002),非耐多药组与敏感组MTB139基因阳性表达率无统计学差异(P=1.000)。结论:新基因MTB139在耐多药结核菌株有高表达,可能与结核耐多药性相关。 Objective:To verify the expression of new gene MTB139 in clinical tuberculosis isolates, and to explore its relevance to TB drug resistance by detecting the differentially expressed MTB139 gene among clinical mycobacterium tuberculosis(M.tuberculosis)isolates with various resistances. Methods:MTB139 gene expressions in 60 clinical isolates(including 9 multidrug resistant isolates,24 non-multidrug resistant isolates and 27 drug sensitive isolates) were detected by semi quantitative reverse transcriptase polymerase chain reaction technique. Results:The expression MTB139 gene was observed in 7(11.67%)out of the 60 study isolates. Statistical analysis revealed that the difference in the expression of MTB139 among the three groups(multidrug resistant group,non-multidrug resistant group,drug sensitive group)was significant(P=0.000). The positive expression rate of MTB139 in multidrug resistant group(5 isolates,55.56%)was significantly higher than that in both non-multidrug resistant group(1 isolates,4.17%)and drug sensitive group(1 isolates,3.70%),with statistical significances(P =0.003;P =0.002). There was no significant difference in the expression of MTB139 gene between non-multidrug resistant group and drug sensitive group(P=1.000). Conclusion:The expression rate of new gene MTB139 is high among the isolates in multidrug resistant group, and MTB139 gene may be associated with multidrug resistance of M.tuberculosis.
