摘要 目的观察分析CT脑灌注成像(CTP)与CT血管成像(CTA)在颈动脉狭窄性短暂性脑缺血发作(TIA)中的诊断意义。方法选取本院2013年6月至2014年6月收治的40例颈动脉狭窄性短暂性脑缺血发作患者,全部患者均接受头颅CT扫描、CT脑灌注成像、CT血管成像检查,对兴趣区、对侧相应区局部脑血容量(rCBV)、局部脑血流量(rCBF)、平均通过时间(MTT)、达到峰值时间(TTP)等指标进行测定,科学评估患者颈动脉狭窄程度、斑块性质。结果病灶侧TTP、MTT、rCBF与对照侧比较差异有统计学意义(P〈0.05),rCBV比较差异无统计学意义(P〉0.05);Ⅰ期、Ⅱ期MTT、rCBF比较差异有统计学意义(P〈0.05),rCBV比较差异无统计学意义(P〉0.05)。结论采用CT各项技术诊断颈动脉狭窄性短暂性脑缺血发作,可科学评价脑组织血流动力学和颈内动脉血管形态变化情况,对疾病的诊断、治疗提供科学参考,具有重要的应用价值。 Objective To investigate the diagnostic value of CT perfusion imaging and CT angiography in the diagnosis of carotid artery stenosis transient ischemic attack (TIA). Methods 40 cases of carotid artery stenosis transient ischemic attack in our hospital from June 2013 to June 2014 were selected. All patients underwent head CT scan, CT cerebral perfusion imaging (CTP) and CT angiography (CTA) inspection, region of interest, on the side of the corresponding regional cerebral blood volume (rCBV), regional cerebral blood flow (rCBF), mean through time (MTT), time to peak (TTP) and other indexes were determined, the scientific evaluation on carotid artery stenosis, plaque properties. Results There were statistically significant differences in TTP, MTT, rCBF between the lesion side and control side (P〈0.05), there were statistically significant differences in MTT and rCBF between phase I and II (P〈0.05). Conclusion CT in the diagnosis of carotid artery stenosis in patients with transient ischemic attack, can scientifically evaluate the hemodynamics of brain tissue and morphological changes of blood vessels of internal carotid artery, provide scientific reference for the diagnosis and treatment of the disease, with important application value.
