老年卧床病人医源性腹泻的原因及护理特点

目的探讨老年卧床病人医源性腹泻原因及护理特点。方法调查2011年3月至2012年9月我院老年病房发生医源性腹泻13例患者的临床资料及护理方法，分析腹泻原因及护理特点，结果（1）医源性腹泻原因：3例为肠内营养液输入，2例为肠道感染性腹泻、6例为抗生素相关性腹泻、1例为机械通气相关性腹泻、1例为胃肠动力药物相关性腹泻。（2）13例医源性腹泻患者预后：1例患者闪失血性休克死亡，余12例患者调整抗生素、使用胃肠调节药后腹泻逐步缓解，总治疗有效率为92．3％（12／13）。结论及时发现、准确评估腹泻病情，给予针对性治疗及护理措施，是预防腹泻并发症的有效方法。

Objective To investigate the causes and nursing characteristics of iatrogenic diarrhea in bed ridden elderly patients. Methods The clinical data and nursing methods of 13 cases of iatrogenic diarrhea in the elderly in our hospital from March 2011 to September 2012 were investigated, and the causes of diarrhea and nursing characteristics were analyzed. Results （1）The causes of iatrogenic diarrhea： 3 cases of enteral nutrition fluid input, 2 cases of intestinal infection diarrhea, 6 cases of antibiotic associated diarrhea, 1 case of mechanical ventilation associated diarrhea, and 1 case of gastrointestinal motility drug associated diarrhea. （2）The prognosis of 13 cases of iatrogenic diarrhea： 1 patient died due to hemorrhagic shock, diarrhea gradually eased in 12 patients after adjusting antibiotics, the use of gastrointestinal conditioning medicine, the total treatment effective rate was 92.3% （12/13）. Conclusion Timely detection and accurate assessment of the condition of diarrhea, and giving specific treatment and nursing measures is an effective way to prevent the complications of diarrhea.
