耳穴贴压结合子午流注时辰疗法对胃癌术后疼痛的影响

Effect of Auricular Points Pressing Combined with TCM Chronotherapy on Postoperative Pain of Gastric Cancer

目的:观察耳穴贴压结合中医基本理论子午流注时辰法对胃癌手术患者术后疼痛的干预效果。方法:将100例行胃癌手术的患者采用随机数字表将患者随机分为对照组和观察组,每组50例。对照组患者给予常规疗方法;观察组在对照组的基础上行耳穴贴压结合时辰疗法治疗。通过测定患者疼痛程度评分、自控镇痛泵(patient controlled analgesia,PCA)的使用量及患者满意度来评价耳穴贴压结合时辰疗法对胃癌术后疼痛的干预效果。结果:观察组在术后48 h内的疼痛程度评分明显低于对照组,差异有统计学意义(P<0.05);在PCA消耗量方面,观察组使用量低于对照组,两组比较,差异具有统计学意义(P<0.05);在胃肠功能恢复方面,观察组胃肠功能恢复时间短于对照组,但两组差异无统计学意义(P>0.05);观察组患者对术后疼痛控制的满意度优于对照组,差异有统计学意义(P<0.05)。结论:耳穴贴压结合子午流注时辰疗法可以有效缓解患者术后疼痛的程度,较少术后止痛药物的使用量,从而提高患者的舒适度及对疼痛控制的满意度。

Objective：To observe the clinical effect of auricular points pressing therapy combined with TCM chronotherapy on the postoperative pain of gastric cancer patients. Methods：100 patients with gastric surgery were randomly divided into control and experiment groups. The control group were treated with conventional treatment methods,while the experiment group was treated with auricular points pressing theory combined with TCM chronotherapy besides the treatment on the control group. By the scores of pain degree of the patient,patient controlled analgesia（ PCA） usage and patient satisfaction,the clinical effect of auricular points pressing theory combined with TCM chronotherapy on the postoperative pain of gastric cancer patients were evaluated. Results：The pain score of the experiment group within 48 h after operation was significantly lower than that of the control group（ P 〈 0. 05）. In terms of PCA consumption,the amount of the experiment group was significantly less than that of the control group（ P 〈 0. 05）. The time taken in gastrointestinal function recovery was shorter than that of the control group,but the difference was not statistically significant（ P 〉 0. 05）. The satisfaction degree of the experiment group of patients in postoperative pain control was higher than that of the control group,and the difference was statistically significant（ P 〈 0. 05）. Conclusion：Auricular points pressing therapy combined with TCM chronotherapy on the postoperative pain of gastric cancer patients can effectively alleviate the degree of postoperative pain,with less postoperative pain medication use,more patient comfort and satisfaction in pain control.
