耳后皮瓣设计联合自体肋软骨移植一期法耳廓再造术矫治Ⅰ度小耳畸形

Microtia of Ⅰ degree retroauricular flap combined with autologous rib cartilage primary transplantation of auricular reconstruction for correcting and repairing microtia of Ⅰ degree

目的探讨Ⅰ度小耳畸形整复矫治。方法充分利用畸形耳,在耳后沟中段设计近似三角形皮瓣,对畸形耳上部软骨进行修整,并将耳廓皮瓣设计戍与耳后皮瓣相对应的对偶皮瓣,取自体肋软骨雕刻成耳廓支架,埋植于耳廓皮瓣内,用耳后沟皮瓣转移覆盖支架,耳后区创面应用滑行皮瓣覆盖,创面较大者,可在肋软骨供区选取皮片移植。结果临床应用6例,耳畸形一次性修复,均正常成活,形态自然,颅耳角度维持良好。结论合理利用耳后皮瓣设计联合自体肋软骨移植耳廓再造术,可一次性对耳廓畸形不算严重的Ⅰ度小耳畸形整复成型,具有创伤小、时间短、花费少,效果可靠、满意的特点。

Objective To introduce a method for correcting mierotia of I degree by one stage. Methods Firstly,to made full use of the malformed ear, approximate triangular flaps in the middle of retroaurieular flap were designed. Secondly,cartilage on the upper part of the ear deformity was repaired and a corresponding flap was designed. Thirdly,an autogenous costal cartilage grat obtained and carved to an auricle frame was implanted into the auricle flap covered with transfered retroaurieular groove flap. Lastly,retroauricular wounds was covered with sliding flap or skin grafts taken from the area of costal cartilage in necessity. Results All the ear deformities in six cases applied with this method were repaired successfully with natural shape and reasonable skull-auricles angle. Conclusion Compared with traditional method,for not serious microtia of I degree retroauricular flap combined with autologous rib cartilage primary transplantation of auricular reconstruction has more advantages such as smaller operative wound ,quicker recovery ,less cost and more satisfactory effect.
