摘要
目的探讨能谱CT单能量成像预判股骨头缺血性坏死后塌陷的价值。方法对62例(83髋)临床证实的股骨头缺血性坏死患者采用能谱CT序列扫描,获得常规混合能量图像和110 ke V的单能量图像,对两种图像分别进行图像质量主观评分,记录股骨头囊变数目和测量对比噪声比(CNR)。结果 110 ke V单能量图像的主观评分、股骨头囊变数目及CNR均高于混合能量图像,两种图像差异有统计学意义(P<0.05)。结论能谱CT单能量成像能够明显优化股骨头缺血性坏死的图像质量,尤其是可以提高股骨头囊变的检出率,有助于前瞻性的预判股骨头坏死后塌陷的风险。
Objective To investigate the value of spectral CT monochromatic imaging in predicting collapse of avascular necrosis of the femoral head. Methods Eighty-three hips of avaseular necrosis of femoral head in 62 pa- tients were scanned by spectral CT imaging to obtain conventional polychromatic images and 110 keV monochromatic images which were respectively graded in image quality. Meanwhile, femoral head cystic changes and CNR were recorded. Results The quality score, cystic changes in femoral head and CNR of 110 keV monochromatic images were higher than those of polychromatic images, it was found that all differences were of statistic significances (P〈 0.05). Conclusion 110 keV monochromatic CT images can significantly optimize the imaging quality of avascular necrosis of femoral head, particularly improving the detection rate of cystic changes in femoral head, can help predict the risk of collapse of avaseular necrosis of femoral head.
