不同促排卵方案在卵巢低反应患者体外受精-胚胎移植的临床应用价值 被引量:18

摘要 目的探讨治疗卵巢低反应(poor ovarian response,POR)患者的最佳促排卵方案。方法回顾性分析接受体外受精-胚胎移植(in-vitro fertilization-embryo transfer,IVF-ET)助孕且诊断为POR患者的247个周期。按照不同的促排卵方案将患者分成自然周期组(35周期)、黄体期促排组(23周期)以及拮抗剂方案组(189周期)三种不同研究组,分析患者基础内分泌水平,比较各组患者的不同促性腺激素(Gn)用量、Gn用药时间、获卵数、成熟卵数、受精率、可用胚胎率、优质胚胎率、周期移植率、移植周期临床妊娠率等相关指标。结果拮抗剂方案组平均Gn用量显著高于黄体期促排卵组。拮抗剂方案组平均Gn使用时间也显著长于黄体期促排组,自然周期组平均获卵数以及获得成熟卵子数,均显著少于黄体期促排卵组和拮抗剂方案组。黄体期促排卵方案组的最终临床妊娠率47.83%,要显著高于自然周期组22.86%及拮抗剂方案组26.46%。结论自然周期费用低,但是治疗效率也低。黄体期促排卵获得了相对较高的助孕结局,但是促排卵周期与移植周期分离,也造成了治疗时间的延长。拮抗剂方案用药费用高,助孕结局较差。
