血流动力学Reference Region模型与Extended Tofts模型在乳腺DCE-MRI定量分析中的应用研究 被引量:4

摘要 目的:探讨两种血流动力学模型及不同参照肌肉的选择对乳腺浸润性导管癌(invasive ductal cancer,IDC)动态增强磁共振成像(DCE-MRI)定量分析参数结果的影响。方法:选取40例经病理证实为乳腺IDC患者,患者术前均行MRI动态增强扫描。动态增强数据导入血流动力学定量分析软件进行处理,首先分别选取胸大肌和肋间外肌作为参照肌肉,并将两者的时间浓度曲线分别运用到Reference Region模型中,得到两组渗透性参数RRKtrans(容积转运常数)和RRKep(速率常数);再运用Extended Tofts模型,以胸主动脉作为动脉输入函数(arterial input funcition,AIF),得到一组Ktrans及Kep参数;最后应用SPSS19.0分别对3组结果进行组内和组间分析。结果:在Reference Region模型中,分别以胸大肌及肋间外肌作为感兴趣区所得的RRKtrans及RRKep之间差异有统计学意义,运用Extended Tofts模型所得参数与Reference Region模型所得参数比较差异有统计学意义。结论:Reference Region模型中选取胸大肌作为感兴趣区可以得到更加稳定的时间浓度曲线;与Extended Tofts模型相比,Reference Region模型能为乳腺DCE-MRI定量分析提供更好的技术基础。
