基于肝病实脾理论研究健脾运脾法对肝硬化失代偿期患者营养状态的影响

Im pacts of Reinforcing Spleen and Activating Spleen Therapy on Nutritional Status of Liver Cirrhosis Patients at Decompensation Stage According to the Theory of Treating Liver by Nourishing Spleen

目的观察"肝病实脾"理论指导下应用健脾运脾方药干预肝硬化失代偿期患者营养状态的疗效。方法采用随机数字表法将入组患者分为治疗组和对照组各60例,对照组予西医常规治疗及营养支持,治疗组加用健脾运脾中药治疗,疗程均为1个月,比较治疗前后营养状况分级、三头肌皮褶厚度(TSF)、上臂肌围(AMC)、体质量指数(BMI)、血清白蛋白(ALB)、前白蛋白(PA)的变化,评价临床疗效。结果治疗后治疗组和对照组在营养状况分级、TSF、AMC、BMI、ALB、PA水平方面均较治疗前明显改善(P<0.05),且治疗组与对照组差异具有统计学意义(P<0.05)。结论基于"肝病实脾"理论,应用健脾运脾方药有利于改善肝硬化失代偿期患者的营养状态。

Objective： To evaluate the effect of reinforcing spleen and activating spleen Chinese herb therapy on nutritional status of liver cirrhosis patients at decompensation stage according to the theory of Treating liver by nourishing spleen. Methods： The enrolled patients were randomized into the treatment group（60 cases） and the control group（60 cases）. The treatment group received routine western medicine treatment along with nutrition support therapy and Chinese medicinal herb treatment,while the control group received routine western medicine treatment along with nutrition support therapy. The treatment lasted for 1 month. Before and after treatment,nutrition classification,triceps skin fold（TSF）,mid-arm muscle circumference（AMC）,body mass index（BMI）,serum albumin（ALB）,prealbumin（PA） were observed to evaluate the efficacy. Results： Nutrition classification,TSF,AMC,BMI,ALB,PA improved significantly after treatment in both groups（P 0.05）. The treatment group was much better than the control group,indicating the statistically significant difference（P 0.05）. Conclusion： Reinforcing spleen and activating spleen herbs can improve nutritional status of liver cirrhosis patients at decompensation stage according to the theory of Treating liver by nourishing spleen.
