氯吡格雷联合阿司匹林治疗社区老年冠心病心绞痛患者疗效观察

Efficacy of community elderly patients with angina pectoris clopidogreland aspirin therapy

目的探讨氯吡格雷联合阿司匹林治疗社区老年冠心病治疗效果改善情况以及服药安全性。方法 120例患者随机分为两组,观察组患者在常规治疗的基础上服用氯吡格雷(75mg/d,1次/d),阿司匹林(0.1mg,1次/d);对照组患者仅口服阿司匹林(0.1mg,1次/d)。比较患者服药期间的心绞痛发作频率、持续时间和硝酸甘油用量,以及患者不良反应和心电图情况。结果观察组心绞痛改善有效率(90.0%)显著高于对照组(78.3%),比较差异具有统计学意义(P<0.05);并且观察组心电图疗效有效率(78.3%)显著高于对照组(63.3%),比较差异具有统计学意义(P<0.05)。以及观察组患者心绞痛频率、硝酸甘油用量、心电图缺血时间和次数均显著少于对照组,两组比较差异具有统计学意义(P<0.05)。在服药过程,两组患者均未见严重的不良反应,轻微不良反应发生率,两组比较差异无统计学意义(P>0.05)。结论氯吡格雷联合阿司匹林治疗老年冠心病心绞痛疗效确切,可以明显改善患者的心绞痛和心电图状况,并且安全性高,值得临床广泛应用推广。

Objective To investigate clopidogrel plus aspirin treatment of coronary heart disease in elderly community, angina pectoris, electrocardiogram and medication safety improvement. Methods 120 patients were randomly divided into two groups, the observation group of patients tookclopidogrel（75mg/d, 1 time/d） on the basis of conventional therapy, aspirin（0.1mg, 1 time/d）; control group of patients only aspirin（0.1mg, 1 time/d）,comparing of patients with angina medication during the attack frequency, duration and amount of nitroglycerin, as well as adverse reactions in patients and electrocardiogram situation. Results The total effective rate of angina improvement was 90.0%; angina patients in the control group to improve the overall response rate was 78.3%. The observation group was significantly higher, the difference was statistically significant（P〈0.05）.The observation group Patients with ECG total effective rate was 78.3%,and the control group of patients ECG total effective rate was 63.3%, observed group was significantly higher, the difference was statistically significant（P〈0.05）. As well as the observation group were angina frequency and nitroglycerin consumption, ischemic electrocardiographic time and frequency were significantly less than the control group, the difference was statistically significant（P〈0.05）. During treatment, patients were no serious adverse events, the incidence of mild adverse reactions difference between the two groups were not statistically significant（P 〉0.05）. Conclusion The clopidogreland aspirin therapy in elderly patients with angina pectoris had a significant effect, the symptoms of angina and ECG efficacy in patients with good improvement, and safe, worthy of wider application promotion.
