血清内脂素和高敏C-反应蛋白水平测定在多囊卵巢综合征中的临床意义 被引量:1

摘要 目的了解血清内脂素和高敏C-反应蛋白水平测定在多囊卵巢综合征中的临床意义,为该疾病的临床诊断提供研究依据。方法 115例PCOS患者根据BMI被分为肥胖PCOS组64例和非肥胖PCOS组51例,另选取同期体检健康的87名妇女作为对照组。检测所有受试者的总胆固醇(TC)、总甘油三酯(TG)、高密度脂蛋白(HDL)、低密度脂蛋白(LDL)、血清睾酮(T)、雌二醇(E2)、卵泡生成激素(FSH)、黄体生成激素(LH)、催乳激素(PRL)、内脂素、hs-CRP和血清胰岛素(INS)水平,并计算胰岛素抵抗指数(HOMA-IR)。结果 PCOS组的T、FSH、LH、PRL水平均高于对照组(P均<0.05)。非肥胖PCOS组、肥胖PCOS组的内脂素、TG、hs-CRP水平均显著高于对照组(t=10.314、7.469、4.469、20.657、12.749、24.107,P均<0.05);非肥胖PCOS组、肥胖PCOS组的HDL水平显著低于对照组(t=1.772、3.214,P均<0.05);肥胖PCOS组的内脂素、TG、hs-CRP水平均显著高于非肥胖PCOS组(t=8.722、5.382、14.799,P均<0.05);肥胖PCOS组的HDL水平低于非肥胖PCOS组(t=1.662,P<0.05)。肥胖PCOS组的空腹血糖、OGTT后2h血糖、空腹INS、OGTT后2h INS和HOMA-IR均高于非肥胖PCOS组(P均<0.05)。内脂素与TG(r=0.415,P<0.05)、hs-CRP(r=0.397,P<0.05)、HOMA-IR(r=0.459,P<0.05)呈正相关,内脂素与HDL(r=-0.295,P<0.05)呈负相关。hs-CRP与空腹INS(r=0.354,P<0.05)、OGTT后2h INS(r=0.260,P<0.05)、HOMA-IR(r=0.426,P<0.05)呈正相关,hs-CRP与HDL(r=-0.327,P<0.05)呈负相关。结论 PCOS患者存在血清内脂素和hs-CRP水平异常,可能与高血脂、胰岛素抵抗有关;血清内脂素、hs-CRP水平可作为评价PCOS患者胰岛素抵抗程度的指标。
