摘要
目的评价HbA2和HbF检测在儿童β-地中海贫血(β-地贫)筛查中的应用价值。方法选取β-地中海贫血患儿98例以及健康儿童105例作为研究对象,分别检测血浆中Hb A2、HbF含量。结果β-地贫组HbA2、Hb F水平明显高于对照组(P<0.01)。HbA2、HbF及联合诊断β-地贫受试者工作特征曲线下面积分别为0.921,0.900,0.951,cutoff值分别为3.3%,0.6%。Logistic回归分析表明,与HbF相比,HbA2诊断价值更大。结论 HbA2和HbF检测对β-地贫诊断具有较高的灵敏度和特异性,是大规模筛查β-地贫比较理想的方法;且HbA2的诊断价值优于HbF;HbA2和HbF联合检测可提高儿童β-地贫的诊断符合率。
Objective To evaluate the application values of hemoglobin A2 / F( Hb A2 and Hb F) detection in β-thalassemia screening among children. Methods A total of 98 children with β-thalassemia and 105 healthy children were selected,the contents of plasma Hb A2 and Hb F were detected respectively. Results The levels of Hb A2 and Hb F in β-thalassemia group were significantly higher than those in control group( P〈0. 01). The areas under ROC of Hb A2,Hb F,and joint detection were 0. 921,0. 900,and 0. 951,respectively; cut-off values were 3. 3% and 0. 6%,respectively. Logistic regression analysis indicated that the diagnostic value of Hb A2 was higher compared with Hb F. Conclusion Hb A2 and Hb F detection have high sensitivity and specificity in β-thalassemia diagnosis,they are ideal methods for large-scale β-thalassemia screening; the diagnostic value of Hb A2 is higher than that of Hb F; joint detection of the two indexes can improve the diagnostic accordance rate of β-thalassemia among chindren.
