2型糖尿病中医证型与动态血糖变化相关性的临床研究

Clinical study on the relationship between TCM syndrome types and dynamic blood glucose in type 2 diabetes mellitus

目的:探讨2型糖尿病(T2DM)不同中医证型与动态血糖变化的相关性,为临床证治提供参考。方法:选取60例T2DM患者,进行中医证型与血糖波动指标的观察,通过观察寻找其相关性。结果:3组受试者24h平均血糖比较,痰(湿)热互结组>热盛伤津组>气阴两虚组;血糖水平的标准差(SDBG)有明显差异,痰(湿)热互结组>热盛伤津组>气阴两虚组(P<0.01);平均血糖漂移幅度、最大血糖波动幅度、日内漂移次数等指标比较,热盛伤津组>痰(湿)热互结组>气阴两虚组(P<0.05,P<0.01)。结论:痰(湿)热互结患者整体血糖水平较高,且三餐后血糖升高较为明显;热盛伤津患者,低血糖现象发生频率较高,且整体血糖波动幅度较大;气阴两虚患者,整体血糖水平偏高,血糖波动差异不显著。

Objective: To investigate the correlation between type 2 diabetes mellitus(T2DM) in different TCM syndromes and blood glucose, and provide reference for clinical treatment. Methods: 60 cases of T2 DM patients were observed, and the TCM syndrome type of blood glucose fluctuation index, through the observation of the correlation. The results of the three groups of subjects 24 h average blood glucose, phlegm stasis group> heat(wet) heat injury group>qi and yin dificiency group; the standard deviation of blood glucose had significant difference(wet), phlegm heat stasis group>heat injury group>qi and yin diffciency group(P<0.01); compared with the mean amplitude of glycemic excursion, the most high blood glucose fluctuations and day shift frequency and other indicators, heat injury group>phlegm heat stasis group(wet)>qi and yin dificiency group(P<0.05); the conclusion of phlegm heat stasis(wet) whole blood glucose level in patients with high blood sugar after meals, and increased more significantly; heat injury Tianjin patients, the frequency of occurrence high hypoglycemia phenomenon, and the whole blood glucose fluctuation in patients with Qi and yin deficiency; the level of blood glucose, blood glucose fluctuation is higher, the difference was not significant.
