“老十针”防治乳腺癌化疗相关恶心呕吐的前瞻性随机对照研究

Prospective randomized controlled trial of treating and preventing chemotherapy-related nausea and vomiting on breast cancer patients with ‘experienced ten acupoints’

目的:探讨"老十针"防治乳腺癌患者发生化疗相关恶心呕吐的临床疗效。方法:本研究通过前瞻性随机对照的研究方法,收集自2014年9月-2016年2月期间确诊乳腺癌且既往化疗期间出现恶心呕吐的患者共64例,随机分为治疗(A)组32例("老十针"+基础药物治疗)和对照(B)组32例(假穴浅刺+基础药物治疗),评价其恶心呕吐程度、中医临床症状及焦虑抑郁状态改善程度。结果:A组化疗期间恶心呕吐程度逐渐下降,化疗第1天和第3天缓解程度均优于B组(P<0.05);两组患者均出现不同程度的食少纳呆、体倦乏力、食后腹胀症状,A组食少纳呆、体倦乏力症状评分第3天时较B组低(P<0.05);A组焦虑及抑郁评分均逐渐下降,化疗第1天及化疗第3天焦虑及抑郁评分均比B组低(P<0.05)。结论:"老十针"可减轻乳腺癌患者恶心呕吐程度,改善食少纳呆、体倦乏力症状,从而减轻患者焦虑抑郁评分,为中医"治未病"提供新思路。

Objective: To observe the clinical effect of ‘experienced ten acupoints' in preventing and treating chemotherapy-related nausea and vomiting of breast cancer patients. Methods: In a prospective randomized control study, 64 breast cancer patients with chemotherapy-related nausea and vomiting from Sep. 2014 to Feb. 2016 were divided into 2 groups. Group A including 32 patients were given the treatments of ‘experienced ten acupoints' and symptomatic therapy. Group B including 32 patients were given the treatments of acupuncturing at sham points and symptomatic therapy. Degree of nausea and vomiting, TCM clinical symptoms, the state of depression and anxiety were observed and analyzed. Results: Relieved degree of nausea and vomiting of group A was better than group B on the first day and the third day(P<0.05). The patients of both groups experienced poor appetite, body tired fatigue and abdominal distention after eating during chemotherapy. The symptom scores of poor appetite and body tired fatigue of group A were lower than that of group B in the third day(P<0.05). Scores of HADS of group A were lower than that of group B on the first day and the third day(P<0.05). Conclusion: Treatments of experienced ten acupoints could help breast cancer patients relieving nausea and vomiting, improving the symptoms of poor appetite and body tired fatigue, and reducing HADS, and providing new way to the TCM ‘preventive treatment of disease'.
