血管源性轻度认知障碍社区筛查情况及中医药干预研究

Study on community screening of vascular mild cognitive impairment and intervention by traditional Chinese medicine

目的:探讨血管源性轻度认知障碍(VMCI)社区筛查情况及中医药效果。方法:选择2015年1月-2016年12月石家庄市、廊坊市、沧州市10个社区年龄在50-80岁中筛查符合VMCI诊断且证型为"痰浊瘀阻,毒损脑络"的患者108例,随机分3组,比较3组患者的疗效及不同时点各项指标的变化。结果:对筛查中符合为"痰浊瘀阻,毒损脑络"的患者年龄进行分层(χ2=13.282)差异有统计学意义;对性别进行分层(χ2=0.915),差异无统计学意义。治疗后,3组患者不同时点MMSE、Mo CA、ADL评分和证候积分比较,差异具有统计学意义(P<0.05)。3组患者不同时点主要症状(Z=2.893)和次要症状(Z=3.102)轻重程度经秩和检验比较,差异具有统计学意义(P<0.05)。结论:应用化浊解毒活血通络法及中医特色治疗对VMCI进行早期干预,效果显著,且丰富了VMCI及预防血管性痴呆防治方法,值得临床推广应用。

Objective: To explore the situation of community screening of vascular mild cognitive impairment(VMCI) and the intervention of traditional Chinese medicine. Methods: One hundred and eight patients between 50-80 years old with the syndrome ‘stagnant blockade of turbid phlegm, toxicity blocking brain' and were corresponding to VMCI from Jan. 2015 to Dec. 2016 in 10 community neighborhood committee in Shijiazhuang, Langfang and Cangzhou were screening out and randomly divided into three groups. The efficacy of three groups and the changes of indicators at different time points were analyzed. Results: Stratified by age(χ2=13.282), the difference in different group is statistically significant. However, stratified by gender(χ2=0.915), the difference is not statistically significant. After treatment, the score of MMSE, Mo CA, ADL and the syndrome integrality in three groups at different time points were significantly different(P<0.05). The cardinal symptom(Z=2.893) and minor symptom(Z=3.102) at different time points in three groups had significant difference after treatment(P<0.05). Conclusion: The clinical effect of traditional Chinese medicine in treating VMCI is significant and it enriches the prevention methods for treating VMCI and vascular dementia. It could be worthy of clinical application.
