摘要
筋骨系统是维持膝关节生物力学平衡的关键,筋在外,为肝所主,是膝关节的稳定体系;骨在内,为肾所主,是膝关节的支撑体系。膝骨关节炎时筋先病变,随着疾病发展,再伤及骨,提示了膝骨关节炎病在筋骨,病位在肝肾,生物力学失衡是筋骨失衡的关键病因。基于膝为筋之府,以膝关节筋骨结构为立足点,以筋骨力学平衡为切入点,结合膝骨关节炎"筋-骨"病机演变规律,归纳膝骨关节炎筋骨失衡的病理特点,提出筋骨互用是指导膝骨关节炎治疗的核心理念,并以此为基础,形成治筋、治骨与筋骨同治的新策略,从而恢复膝关节的筋骨力学平衡。
The Jin-Gu system plays an important role in maintaining the biomechanics balance of knee joint. Jin in the outside as stable system of knee joint is governed by the liver, Gu in the inside as support system of knee joint is governed by the kidney. Jin lesions occur first and then Gu lesions with the development of knee osteoarthritis(KOA), indicating that KOA is characterized by the pathological change of Jin-Gu and the disease located in liver and kidney, and the biomechanics imbalance cause to the Jin-Gu imbalance. Based on knee joint as the house of Jin and the biomechanics imbalance of Jin-Gu, it is put forward that the Jin-Gu interoperate as core idea guides the treatment of KOA combined with pathogenesis evolution law of Jin-Gu and pathological features of Jin-Gu imbalance in the process of KOA. It is a new treatment strategy of KOA that treating from the JinGu theory are to recover thebiomechanics balance of knee joint.
