同仁堂舒肝丸联合防风通圣丸治疗围绝经期抑郁肥胖临床观察

Clinical observation of Tongrentang Shugan Pill combined with Fangfeng Tongsheng Pill in treatment of menopausal depression with obesity

目的:探讨同仁堂舒肝丸与防风通圣丸联合方案在围绝经期抑郁肥胖治疗中的临床应用效果。方法:选择2014年1月-2016年1月北京同仁堂中医医院收治的围绝经期抑郁并肥胖患者作为研究对象,共计70例。根据随机数表法将70例患者平均分为两组,即研究组与对照组,各35例。对照组应用安慰剂、氟西汀及针灸治疗,研究组在对照组的基础上应用同仁堂舒肝丸联合防风通圣丸治疗。结果:研究组治疗的总有效率为88.57%,对照组为62.86%,研究组总有效率高于对照组(P<0.05)。两组治疗前BMI指数、体质量及HAMD评分对比差异无统计学意义;治疗后两组患者BMI指数、体质量及HAMD评分较治疗前均显著降低,研究组治疗后BMI指数、体质量及HAMD评分均低于对照组(P<0.05)。两组治疗前FSH与E2水平对比差异无统计学意义;治疗后两组患者FSH低于治疗前、E2高于治疗前(P<0.05),且研究组治疗后FSH水平低于对照组(P<0.05),E2水平高于对照组(P<0.05)。结论:同仁堂舒肝丸联合防风通圣丸治疗围绝经期抑郁肥胖效果显著,可以有效调节体质量,改善不良心理状况,适于临床应用。

Objective: To study the effect of Tongrentang Shugan Pill combined with Fangfeng Tongsheng Pill in the treatment of menopausal depression with obesity. Methods: Seventy cases of menopausal depression with obesity in Beijing Tongrentang Hospital of TCM from January 2014 to January 2016 were selected. According to the random number table method, the patients were divided into treatment group and control group, each of 35 cases. The control group was treated with placebo, fluoxetine and acupuncture, the treatment group was treated with Tongrentang Shugan Pill combined with Fangfeng Tongsheng Pill on the basis of the control group. Results: The total effective rate of the treatment group was 88.57%, and the control group was 62.86%. The total effective rate of the treatment group was higher than that of the control group(P<0.05). The clinical efficacy of the treatment group was significantly better than the control group(P<0.05), the difference of BMI index, body weight and HAMD score between the two groups before treatment had no significant differences; after treatment of patients with BMI index, two groups of body weight and HAMD score were significantly lower than before, BMI index, body weight and HAMD score of the treatment group were lower than the control group(P<0.05). The difference between the FSH and the E2 level of the two groups had no statistical significance before treatment; two patients in FSH group and E2 group after treatment level has lower than that before, FSH levels in the treatment group after treatment was lower than that of control group(P<0.05), E2 was higher than the control group(P<0.05). Conclusion: Effect of Tongrentang Shugan Pill combined with Fangfeng Tongsheng Pill in the treatment of menopausal depression with obesity is significant, can be effectively in regulating body weight, improve the adverse psychological status, and suitable for clinical application.
