复方隔药灸的现代临床应用分析

Analysis on clinical application of compound separated-herb moxibustion

目的:对近30年组方符合君臣佐使的配伍原则,至少包含5味中药的复方隔药灸的临床应用文献进行分析,阐释其临床应用现状及优势,为临床诊治疾病提供思路。方法:检索国内中国知网、万方数据库、重庆维普数据库及中国生物医学文献数据库,国外Pubmed、Medline及EMBAS数据库1985年-2016年收录的关于复方隔药灸的临床研究文献,采用文献计量学分析方法对其穴位、复方药物使用情况,疾病谱分别进行统计分析。结果:根据检索到的复方隔药灸文献,经阅读摘要、关键词、全文,最终确定符合标准的临床文献共177篇,涉及病症61种,原发性痛经、腹痛、腹泻、肠易激综合征和溃疡性结肠炎是其治疗的优势病种;涉及穴位78个,神阙穴使用频次最多,其次为足三里、关元、中脘、气海、肾俞、天枢和脾俞穴,均在20次以上;复方药物组成以5味药、6味药居多,以肉桂、附子和当归作为君药,木香、丹参和红花作为臣药,乳香、没药和冰片作为佐药组成的复方较多。结论:复方隔药灸辨证选药,随证取穴,具有广泛的适应症,且疗效突出,这一绿色疗法正在日益受到人们的关注。

Objective: To elucidate the clinical advantages of compound moxibustion by analyzing the principle of compatibility of prescriptions in accordance with the monarch for compound moxibustion including at least 5 traditional Chinese medicines in recent 30 years, as so to provide ideas for the clinical diagnosis and treatment. Methods: The clinical research literature about moxibustion were retrieved from CNKI, Wanfang database, VIP database, Chinese biomedical literature database, Pubmed, Medline, and EMBAS from 1985 to 2016. The acupoints, herbs use and disease spectrum were analyzed respectively by the bibliometric method. Results: A total of 177 literatures were finally included into this study. The 61 kinds of diseases were involved, in which the primary dysmenorrhea, abdominal pain, diarrhea, irritable bowel syndrome and ulcerative colitis were well treated as dominant disease; The 78 acupoints were involved, in which the Shenque(RN8) were the most frequently used, followed by Zusanli(ST36), Guanyuan(RN4), Zhongwan(RN12), Qihai(RN6), Shenshu(BL23) and Pishu(BL20) that were used for more than 20 times; The prescriptions were commonly composed of the sovereign drugs including the Cortex Cinnamomi, Radix Aconiti Lateralis Preparata and Radix Angelicae Sinensis, the minister drugs including Radix Aucklandiae, Radix Aucklandiae and Flos Carthami, and assistant drugs including Olibanum, Myrrha and Borneolum Syntheticum, a total of 5 herbs and 6 herbs. Conclusion: According to the prescription and selection acupoints based on syndrome differentiation, the compound separatedherb moxibustion has a wide application and good clinical efficacy, so it is is increasingly concerned by the people.
