功能性腹泻中医辨证标准的现代文献研究

Modern literature research on TCM syndrome differentiation standard of functional diarrhea

目的:通过现代临床文献研究,探索功能性腹泻的中医证候分布规律及其四诊信息情况。方法:检索中国知网、万方数据库、维普数据库、中国生物医学数据库以及Pubmed、Cochrane图书馆临床对照试验资料库,制定纳入排除标准,筛选功能性腹泻中医横断面调查、病例对照研究、疗效观察等文献,提取证候名称及最小单元的中医四诊信息等资料,进行描述性统计。结果:功能性腹泻中医观察性研究及干预性研究共80篇,提及中医证候的文献有42篇,主要涉及10个中医证型,出现频率最高的5个证型为脾胃虚弱、肝郁脾虚、脾肾两虚、寒热错杂、脾虚湿阻,腹泻为共有症状,涉及脾、胃、肝、肾4个病位,以及气虚、气滞、阳虚、寒、热、湿等病性。结论:随着研究的深入,功能性腹泻的中医辨证标准越来越凸显"脾失健运"这一核心病机。

Objective: To clarify the distribution of TCM syndromes of functional diarrhea and its four diagnostic methods information through the modern clinical literature research. Methods: Searching the CNKI, WF, VIP, CBM, PubMed database, and Cochrane library clinical controlled trial database, developing the inclusion of exclusion criteria, screening of functional diarrhea TCM cross-sectional investigation, case-control study, efficacy observation and other literatures, extracting the syndrome names and the minimum unit of Chinese medicine four diagnosis methods information and other information for descriptive statistics. Results: There were 80 observational and interventional studies on functional diarrhea in Chinese medicine,42 of which referred to TCM syndromes, mainly involving 10 TCM syndromes, and the five most frequently occurring syndromes were the syndrome of deficiency of the spleen and stomach, stagnation of liver qi and spleen deficiency, spleen-kidney deficiency,intermingled cold with heat, dampness stagnancy due to spleen deficiency, among which diarrhea was the common symptom, and involving the disease location of spleen, stomach, kidney and liver, and the nature of disease of qi deficiency, qi stagnation, yang deficiency, cold, heat, and dampness. Conclusion: With the deepening of research, the standard of TCM syndrome differentiation of functional diarrhea has increasingly highlighted the core pathogenesis of ‘dysfunction of spleen in transportation’.
