壮医经筋综合疗法治疗腰椎间盘突出症的临床观察及理论探讨

Clinical observation and theoretical study of comprehensive muscle-region therapy in Zhuang medicine for lumbar disc herniation

目的:观察壮医经筋疗法治疗腰椎间盘突出症(LDH)的临床疗效,从而探究其治疗LDH的机制。方法:将80例诊断为LDH患者随机分为治疗组和对照组,各40例,治疗组采用壮医经筋综合疗法(经筋推拿手法、壮医火针刺法及拔罐)治疗,对照组采用传统常规推拿和针灸治疗。于治疗前、后评价患者VAS评分、JOA评分以及胫神经、腓总神经传导速度,比较两组临床疗效。结果:两组患者治疗2个疗程后VAS评分、JOA评分和神经传导速度与同组治疗前比较,差异均具有统计学意义(P<0.05)。治疗组治疗2个疗程后VAS评分、JOA评分和患肢腓总神经传导速度优于对照组(P<0.05)。治疗组总有效率为95.0%,对照组为80.0%,两组比较,差异有统计学意义(P<0.05)。结论:壮医经筋疗法治疗LDH疗效显著,值得临床推广。

Objective:To observe the clinical efficacy of comprehensive muscle-region therapy based on Zhuang medicine in treating lumbar disc herniation(LDH),and to explore the mechanism of treating LDH.Methods:A total of 80 patients with LDH were randomized into a treatment group and a control group,40 cases in each group.The treatment group was intervened by the comprehensive muscle-region therapy based on Zhuang medicine(muscle-region tuina,fire-needle acupuncture of Zhuang medicine and cupping),while the control group was by regular traditional tuina and acupuncture treatment.The Visual Analogue Scale(VAS)score and Japanese Orthopaedic Association scores(JOA)and tibial nerve,common peroneal nerve conduction velocity were observed before and after 2 treatment courses,and the clinical efficacy were compared.Results:The VAS scores and JOA scores and nerve conduction velocity of the treatment group and the control group were significantly different from that before intervention after 2 treatment courses(P<0.05).After 2 treatment courses,the VAS score and JOA scores and nerve conduction velocity in the treatment group was significantly different from that in the control group(P<0.05).The total effective rate was 95.0%in the treatment group versus 80.0%in the control group,and the difference was statistically significant(P<0.05).Conclusion:In clinical treatment,the comprehensive muscle region therapy in Zhuang medicine has a better treatment effect in treating LDH,which is worth to promote this treating method in clinical.
