加味川芎平喘合剂治疗慢性阻塞性肺疾病稳定期合并肺动脉高压的临床疗效观察

Clinical observation of modified Chuanxiong Pingchuan Mixture in treating stable phase of chronic obstructive pulmonary disease complicated with pulmonary hypertension

目的:观察加味川芎平喘合剂对慢性阻塞性肺疾病(COPD)稳定期合并肺动脉高压(PH)痰瘀阻肺型患者的临床疗效及其对凝血因子的影响。方法:将80例COPD稳定期合并PH患者随机分为2组:对照组给予西医常规治疗,治疗组在西医常规治疗基础上给予加味川芎平喘合剂,疗程均为12周。观察并记录两组患者治疗前后临床疗效变化、肺动脉压力、凝血等相关检测指标变化情况。结果:治疗组临床疗效显著优于对照组(P<0.05)。治疗后,治疗组咳嗽、喘息、胸闷、气短等中医症状积分显著低于对照组(P<0.05),肺动脉压力较治疗前显著下降(P<0.05);FIB、D-二聚体水平均较治疗前显著下降(P<0.05),且低于同期对照组(P<0.05)。结论:加味川芎平喘合剂可改善COPD稳定期合并PH患者的临床症状,可一定程度降低患者肺动脉压力,并有效降低FIB、D-二聚体水平。

Objective:To observe the clinical effect of modified Chuanxiong Pingchuan Mixture on patients with stable phase of chronic obstructive pulmonary disease(COPD)complicated with pulmonary hypertension(PH)of phlegm and blood stasis resistance lung syndrome and its effect on blood coagulation factors.Methods:Eighty patients with stable phase of COPD who had PH were randomly divided into two groups.The control group was given routine western medicine treatment.The treatment group was given modified Chuanxiong Pingchuan Mixture on the basis of routine western medicine treatment,the course of treatment was 12 weeks.The changes of clinical efficacy,pulmonary artery pressure,coagulation,and other related indicators before and after treatment in the two groups of patients were observed and recorded.Results:The clinical efficacy of the treatment group was better than that of the control group,with significant differences(P<0.05).After treatment,the total scores of symptoms such as cough,wheezing,chest tightness,and shortness of breath in the treatment group were significantly lower than those in the control group(P<0.05),pulmonary arterial pressure in the treatment group was significantly lower than that before treatment(P<0.05);FIB and D-dimer levels were significantly lower than that before treatment(P<0.05).And the levels of FIB and D-dimer in the treatment group were significantly lower than those in the control group(P<0.05).Conclusion:Modified Chuanxiong Pingchuan Mixture can improve the clinical symptoms of stable phase of COPD in patients with PH,and can reduce the pulmonary artery pressure to a certain extent and effectively reduce the levels of FIB and D-dimer.
