谈复发性阿弗他口腔溃疡的中医脏腑辨证

Discussion on treatment of recurrent aphthous ulcer of syndrome differentiation of zang-fu viscera

复发性阿弗他溃疡(RAU)是一种常见的口腔黏膜病,其患病率居口腔黏膜病的首位,因其发病机制复杂,临床表现具有反复性和周期性等特点,为其治疗带来困难。脏腑辨证尤为适合在杂病中应用,笔者在《黄帝内经》理论指导下,结合中医脏腑经络理论,提出'五脏六腑皆令口腔溃疡,非独心火'的整体辨证观;以及'病在舌,以治心为主;病在唇、颊,以调治脾胃为主'等局部辨证观;并查阅古今文献,梳理出体现中医脏腑辨证治疗口腔溃疡的六大主方,以期更好地继承和发扬中医学,指导临床实践。

Recurrent aphthous ulcer(RAU)is a common oral mucosal disease.Its prevalence ranks first among oral mucosal diseases.Because its complex pathogenesis and clinical manifestations have the characteristics of repetition and periodicity,which brings a difficulty to its treatment.Syndrome differentiation of zang-fu viscera is especially suitable for the treatment of miscellaneous diseases.Guiding by the theory of Huangdi Neijing and Zang-fu meridians and collaterals of traditional Chinese medicine,this paper put forward the holism of syndrome differentiation of‘oral ulcer has relationship with all the internal organs of the body,not only single heart fire’and the local concept of syndrome differentiation of‘when disease locates in the tongue,giving priority to the treatment of the heart;when disease locates in the cheek and lip,giving priority to the treatment of the spleen and stomach’.In addition,by field research,this study combed out the six main formulas for oral ulcer based on syndrome differentiation of zang-fu viscera,in order to better inherit and develop the traditional Chinese medicine and guide clinical practice.
