中药熏洗配合冲击波治疗膝骨关节炎合并鹅足滑囊炎的临床观察

Clinical observation on treatment of knee osteoarthritis combined with goose foot bursitis by Chinese herbal fumigation and shock wave

目的:观察中药熏洗配合体外冲击波联合治疗膝骨关节炎(KOA)合并鹅足滑囊炎的临床疗效。方法:将86例KOA合并鹅足滑囊炎患者按Kellgren和Lawrence标准Ⅱ级、Ⅲ级随机分入治疗组和对照组,两组各43例,治疗组中药祛瘀通痹汤熏洗联合冲击波治疗,对照组仅用中药祛瘀通痹汤熏洗,观察两组患者治疗前后治疗效果评定、VAS疼痛评分和ROM活动度的变化,按照Shahariarre疗效评定标准评定治疗效果。结果:治疗后两组患者的VAS评分显著降低(P<0.01),治疗组优于对照组(P<0.01);治疗后两组患者ROM活动度显著改善(P<0.01),组间比较无统计学差异。结论:中药祛瘀通痹汤熏洗配合冲击波治疗KOA合并鹅足滑囊炎,VAS评分降低优于单纯中药祛瘀通痹汤熏洗,但两组ROM活动度改善无显著差异。

Objective:To observe the clinical effect of traditional Chinese medicine fumigation and washing with extracorporeal shock wave in the treatment of knee osteoarthritis(KOA)combined with goosefoot bursitis.Methods:Eighty-six cases of KOA with goose foot slippery bursa phlogistic patients were selected according to the standard II and III Kellgren and Lawrence,then randomly assigned into the treatment group and control group with 43 cases in each group,the treatment group used Quyu Tongbi Decoction with shock wave therapy,and the control group treated with only Chinese medicine fumigation certification by Quyu Tongbi Decoction.The VAS pain score and ROM activity changes were observed before and after treatment.The therapeutic effect was evaluated according to the Shahariarre therapeutic effect evaluation standard.Results:After treatment,the VAS scores of patients in the two groups were significantly reduced(P<0.01),and the treatment group was superior to the control group(P<0.01).After treatment,the ROM activity of patients in the two groups were significantly improved(P<0.01).Conclusion:In the traditional Chinese medicine fumigation and washing method of Quyu Tongbi Decoction combined with shockwave in the treatment of knee osteoarthritis combined with goose-foot bursitis,the reduction of VAS score is better than traditional Chinese medicine fumigation and washing of Quyu Tongbi Decoction alone,but there is no difference in the improvement of ROM activity between the two groups.
