针灸治疗对肩袖损伤运动功能康复的临床研究

Clinical study on acupuncture treatment for functional rehabilitation of rotator cuff injury

目的:观察针灸治疗对肩袖损伤的临床疗效,进一步证明针灸治疗对肩袖损伤的治疗作用。方法:将214例轻中度肩袖损伤患者根据有无进行针灸治疗分成两组,试验组采用针灸配合物理康复锻炼,对照组只进行物理康复训练。分别于治疗前(TⅠ)、治疗后3周(TⅡ)、治疗后6周(TⅢ)、治疗后12周(TⅣ)时记录两组患者的疼痛视觉模拟评分(VAS)、压力疼痛阈值(PPT)、简明肩关节功能测试(SST)、CMS功能评分(CMS)及治疗相关不良反应,评价针灸对轻中度肩袖损伤的临床疗效。结果:两组患者在治疗前(TⅠ)各项指标差异无统计学意义。治疗后3周(TⅡ)、治疗后6周(TⅢ),试验组VAS、PPT、SST、CMS评分均优于对照组(P<0.05)。TⅣ时,试验组VAS、PPT与对照组无统计学差异,试验组SST、CMS优于对照组(P<0.05)。两组患者在治疗周期内均未发生需要治疗的并发症。结论:针灸配合物理康复锻炼对肩袖损伤的疼痛缓解及功能恢复安全有效。

Objective:To evaluate the efficacy of acupuncture for rotator cuff injury,and further to prove the therapeutic effect of acupuncture treatment on rotator cuff injury.Methods:We retrospectively reviewed patients who suffered from rotator cuff injury in our hospital.All of them were divided into 2 groups in terms of whether acupuncture had been used with physical therapy,acupuncture group received combined acupuncture and standardized physical exercise,while the control group received standardized physical exercise alone.The pain visual analogue scale(VAS),pressure pain threshold(PPT),simple shoulder test scores(SST),Constant-Murley Score(CMS)and complications were compared between the 2 groups before treatment(T I),at 3-week follow-up after treatment(T II),at 6-week follow-up after treatment(T III)and 12-week follow-up after treatment(T IV).Results:The mean follow-up was 10 months.There was no statistic difference between 2 groups at T I.In T II and T III groups,the VAS,PPT,SST and CMS were superior than those in the control group(P<0.05).Regarding VAS and PPT,SST and CMS of acupuncture group performed better than control group in T IV(P<0.05).No severe complication happened during the treatment.Conclusion:Acupuncture combined with standardized physical exercise is an effective and safety treatment for improving pain and functional scores for rotator cuff injury.
